Description

Indications

Xcel is commonly prescribed for a wide range of conditions, including fever, common cold, influenza, headache, toothache, earache, muscle pain, body aches, neuralgia, dysmenorrhoea (menstrual pain), sprains, colic pain, back pain, post-surgical pain, postpartum pain, inflammatory pain, and pain following vaccinations in children. It is also effective for treating rheumatic pain, osteoarthritis, and joint stiffness.

Pharmacology

Paracetamol, the active ingredient in Xcel, is known for its analgesic (pain-relieving) and antipyretic (fever-reducing) properties, with mild anti-inflammatory effects. It is believed to work primarily within the central nervous system (CNS), raising the pain threshold by inhibiting COX-1, COX-2, and COX-3 enzymes involved in prostaglandin synthesis. Paracetamol is a widely used, safe, and fast-acting analgesic with minimal side effects compared to aspirin.

Dosage & Administration

Tablet:

  • Adults: 1-2 tablets every 4 to 6 hours, not exceeding 4 grams (8 tablets) per day.
  • Children (6-12 years): ½ to 1 tablet 3 to 4 times daily. For long-term use, do not exceed 2.6 grams/day.

Extended Release Tablet:

  • Adults & Children (12 years and above): 2 tablets, swallowed whole, every 6 to 8 hours. Do not exceed 6 tablets in 24 hours.
  • The tablet must not be crushed.

Syrup/Suspension:

  • Children under 3 months: 10 mg/kg body weight (or 5 mg/kg if jaundiced) 3 to 4 times daily.
  • 3 months to 1 year: ½ to 1 teaspoonful 3 to 4 times daily.
  • 1-5 years: 1-2 teaspoonfuls 3 to 4 times daily.
  • 6-12 years: 2-4 teaspoonfuls 3 to 4 times daily.
  • Adults: 4-8 teaspoonfuls 3 to 4 times daily.

Suppository:

  • Children 3-12 months: 60-120 mg, 4 times daily.
  • Children 1-5 years: 125-250 mg, 4 times daily.
  • Children 6-12 years: 250-500 mg, 4 times daily.
  • Adults & children over 12 years: 0.5-1 gm, 4 times daily.

Paediatric Drops:

  • Up to 3 months: 0.5 ml (40 mg)
  • 4-11 months: 1.0 ml (80 mg)
  • 1-2 years: 1.5 ml (120 mg)
  • Maximum of 5 doses per day for up to 5 days.

Paracetamol Tablet with Actizorb Technology:
This technology allows the tablet to dissolve up to five times faster than standard paracetamol tablets. It is a fast-acting and safe analgesic, ideal for those who cannot tolerate aspirin or other similar medications.

  • Adults & Children (12 years and over): 1-2 tablets every 4 to 6 hours, with a maximum of 8 tablets in 24 hours.
  • Children (7-11 years): ½ to 1 tablet every 4 to 6 hours, not exceeding 4 tablets in 24 hours.
  • Not recommended for children under 7 years.

Interaction

Patients who regularly consume alcohol, take barbiturates, tricyclic antidepressants, or certain other medications may experience a reduced ability to metabolize Xcel. Alcohol can increase the risk of liver toxicity in case of an overdose. Long-term use of anticonvulsants or oral contraceptives may also interfere with the metabolism of Xcel.

Contraindications

Xcel should not be used by individuals with known hypersensitivity (allergy) to paracetamol.

Side Effects

While Xcel is generally well-tolerated, some individuals may experience mild side effects such as skin rashes, pancreatitis, or allergic reactions. Rarely, blood disorders like thrombocytopenia, leucopenia, and neutropenia have been reported.

Pregnancy & Lactation

Studies have shown no harmful effects of paracetamol when used during pregnancy at the recommended doses. However, it is always advisable to follow the guidance of your healthcare provider. Paracetamol is excreted in breast milk, but not in significant amounts, and is considered safe for breastfeeding mothers.

Precautions & Warnings

Care should be taken when administering Xcel to patients with severe kidney or liver impairment. The risk of overdose is higher in individuals with alcoholic liver disease. Never exceed the recommended dosage, and avoid taking multiple products containing paracetamol simultaneously.

Overdose Effects

Overdose with Xcel can lead to liver damage, particularly in individuals who consume alcohol or are on medications that induce liver enzymes (e.g., Carbamazepine, Phenobarbitone, or Phenytoin). Symptoms of overdose within the first 24 hours include pallor, nausea, vomiting, and abdominal pain. Liver damage may become evident within 12-48 hours. In severe cases, liver failure, encephalopathy, and even death may occur. Immediate medical attention is required.
