Indications:

Cavicet is indicated for the following conditions:

• Acute Cerebrovascular Accidents (Strokes): Effective for ischemic strokes caused by cerebral thrombosis, embolism, or acute circulatory disorders. It also helps in hypertensive crises, complete strokes, multi-infarct dementia, cerebral arteriosclerosis, hypertensive encephalopathy, and post-apoplectic conditions, including hemorrhagic strokes.
• Senile Disorders: Alleviates psychosomatic symptoms associated with cerebral insufficiency in the elderly, such as memory issues, forgetfulness, slow cognitive processes, concentration difficulties, dizziness, mood instability, aphasia, sleep disturbances, and vasovegetative symptoms of menopause.
• Visual Disorders: Treats vascular issues of the retina and choroid caused by arteriosclerosis, vasospasm, macular degeneration, arterial or venous thrombosis, or embolism. Effective for secondary glaucoma resulting from these conditions.
• Hearing Disorders: Used in the treatment of hearing impairment due to vascular or toxic (iatrogenic) causes, such as presbycusis, Meniere’s disease, cochleovestibular neuritis, tinnitus, and dizziness of labyrinth origin.

Pharmacology:

Vinpocetine, the active ingredient in Cavicet, enhances cerebral metabolism by increasing glucose and oxygen consumption, improving tolerance to cerebral hypoxia. It shifts glucose metabolism toward more efficient aerobic pathways, boosting ATP levels in the brain.

It also improves cerebral microcirculation by inhibiting platelet aggregation, reducing high blood viscosity, and enhancing erythrocyte deformability. Vinpocetine promotes better oxygen transport by lowering erythrocyte oxygen affinity.

By selectively increasing cerebral blood flow, Vinpocetine helps reduce cerebral vascular resistance without affecting systemic circulation, ensuring better blood supply to ischemic areas while maintaining blood flow to healthy regions. This mechanism is known as the inverse steal effect.

Dosage & Administration:

• Tablet: 1-2 tablets, 3 times daily. For maintenance, take 1 tablet, 3 times daily.
• IM Injection: 20-40 mg daily until symptoms improve (maximum of 10 days). Afterward, switch to oral treatment. If this doesn’t provide relief, start infusion therapy.
• IV Infusion: Begin with a daily dose of 20 mg in slow infusion (2 ampoules in 500-1000 ml solution). Increase to 1 mg/kg body weight over 3-4 days. Treatment duration: 10-14 days, tapering the dose before discontinuation.

Interaction:

• Incompatible with heparin: Do not mix Cavicet injection with heparin in the same syringe.

Contraindications:

• Parenteral Treatment: Contraindicated in patients with severe ischemic heart disease, severe rhythm disorders, and during pregnancy.

Side Effects:

• Common side effects include transient hypotension and tachycardia.

Pregnancy & Lactation:

• Contraindicated during pregnancy and lactation due to potential risks to the fetus or nursing infant.

Precautions & Warnings:

• Acute cases: Start with parenteral treatment until symptom improvement, then switch to oral treatment.
• Chronic cases: Oral therapy is preferred.

Therapeutic Class:

• Cerebral vasodilator and neurosensory oxygenator drugs.

• Store in a cool, dry place, protected from light and moisture. Keep away from children.
