Vilanterol Trifenatate + Fluticasone Furoate is a combination medication used for long-term maintenance treatment of airflow obstruction and reducing exacerbations in patients with chronic obstructive pulmonary disease (COPD), as well as for once-daily treatment of asthma in adults aged 18 years and older. It contains a selective, long-acting beta-2 agonist (Vilanterol Trifenatate) and a corticosteroid (Fluticasone Furoate), which work together to improve lung function and reduce inflammation in the airways. This medication is not intended for relief of acute bronchospasm.

Indications
- Long-term, once-daily maintenance treatment of airflow obstruction and reducing exacerbations in patients with chronic obstructive pulmonary disease (COPD).
- Once-daily treatment of asthma in patients aged 18 years and older.
- Important limitation of use: Not indicated for relief of acute bronchospasm.

- 25/100 inhalation capsule: Vilanterol 25 mcg (as Vilanterol Trifenatate INN) & Fluticasone Furoate INN 100 mcg.
- 25/200 inhalation capsule: Vilanterol 25 mcg (as Vilanterol Trifenatate INN) & Fluticasone Furoate INN 200 mcg.
Description
- Vilanterol Trifenatate: Selective, long-acting beta-2 agonist.
- Fluticasone Furoate: Corticosteroid with mainly glucocorticoid activity, exerting a topical effect on the lungs without systemic effects at usual doses.
Pharmacology
- Combination of vilanterol and fluticasone furoate.
- Vilanterol: Stimulates intracellular adenyl cyclase, leading to bronchial smooth muscle relaxation.
- Fluticasone furoate: Wide-ranging anti-inflammatory actions on various cell types and mediators involved in inflammation.
Dosage & Administration
- Oral inhalation only, not for swallowing.
- 1 inhalation of 25/100 mcg once daily for COPD maintenance treatment.
- 1 inhalation of 25/100 mcg or 25/200 mcg once daily for asthma treatment.
Interaction
- Strong cytochrome P450 3A4 inhibitors (e.g., ketoconazole): Use with caution.
- Monoamine oxidase inhibitors and tricyclic antidepressants: Extreme caution advised.
- Beta-blockers: Caution recommended.
- Diuretics: Use with caution, monitor for electrocardiographic changes and hypokalemia.
Contraindications
- Primary treatment of status asthmaticus or acute episodes of COPD or asthma.
- Severe hypersensitivity to milk proteins or any ingredients.
Side Effects
- Nasopharyngitis, oral candidiasis, headache, upper respiratory tract infection, bronchitis, sinusitis, cough, among others.
Use in Special Populations
- Not indicated for use in children and adolescents.
- Monitor patients with hepatic impairment for systemic corticosteroid effects.
