Indications
Vanprox is prescribed for treating infections caused by susceptible microorganisms, including:
- Acute otitis media caused by Streptococcus pneumoniae, Streptococcus pyogenes, Haemophilus influenzae, and Moraxella catarrhalis (including beta-lactamase producing strains).
- Pharyngitis/tonsillitis caused by Streptococcus pyogenes.
- Acute maxillary sinusitis caused by Haemophilus influenzae (including beta-lactamase producing strains), Streptococcus pneumoniae, and Moraxella catarrhalis.
- Community-acquired pneumonia caused by S. pneumoniae or H. influenzae (including beta-lactamase-producing strains).
- Acute bacterial exacerbation of chronic bronchitis caused by S. pneumoniae, non-beta-lactamase-producing H. influenzae, or M. catarrhalis.
- Skin and skin structure infections caused by Staphylococcus aureus and Streptococcus pyogenes.
- Uncomplicated urinary tract infections caused by E. coli, Klebsiella pneumoniae, Proteus mirabilis, or Staphylococcus saprophyticus.
- Uncomplicated gonorrhea caused by Neisseria gonorrhoeae (including penicillinase-producing strains).
- Rectal gonococcal infections in women due to Neisseria gonorrhoeae (including penicillinase-producing strains).
Pharmacology
Cefpodoxime, an oral third-generation cephalosporin, demonstrates stability against beta-lactamases and effective activity against a broad spectrum of Gram-negative and Gram-positive bacteria. It is a prodrug that converts to its active form, Cefpodoxime, in the body. Approximately 29 to 33% of Cefpodoxime is excreted unchanged in the urine within 12 hours.
Cefpodoxime is indicated for treating infections in patients infected with susceptible strains of microorganisms, encompassing a wide range of gram-positive and gram-negative bacteria. Its stability against beta-lactamase enzymes makes it particularly effective against gram-positive bacteria compared to other third-generation oral cephalosporins.
Dosage & Administration
For Adults and Adolescents (13 years and older):
- Pharyngitis/tonsillitis: 100 mg every 12 hours for 5 to 10 days.
- Acute maxillary sinusitis: 200 mg every 12 hours for 10 days.
- Community-acquired pneumonia: 200 mg every 12 hours for 14 days.
- Acute bacterial exacerbations of chronic bronchitis: 200 mg every 12 hours for 10 days.
- Skin and skin structure infections: 400 mg every 12 hours for 7 to 14 days.
- Uncomplicated urinary tract infection: 100 mg every 12 hours for 7 days.
- Uncomplicated gonorrhea: single dose of 200 mg.
- Rectal gonococcal infections in women: single dose of 200 mg.
For Infants and Pediatric Patients (2 months to 12 years), the dosages vary based on body weight and specific conditions.
Interaction
Vanprox may interact with antacids, H2 blockers, probenecid, and nephrotoxic drugs, affecting its absorption and excretion. Close monitoring is recommended when administering these medications concomitantly.
Contraindications
Cefpodoxime Proxetil should not be used in patients with known allergies to Cefpodoxime or cephalosporin antibiotics.
Side Effects
Common side effects of Vanprox include diarrhea, nausea, fungal infections of the skin and vagina, abdominal pain, headache, and dizziness. These effects may vary in severity between adults and children.
Pregnancy & Lactation
While Cefpodoxime was not teratogenic or embryocidal in animal studies, its safety during pregnancy has not been adequately studied. Use during pregnancy should be determined based on clinical need, and caution should be exercised during lactation as Cefpodoxime is excreted in human milk.
Precautions & Warnings
Patients with renal impairment should receive adjusted dosages of Vanprox, and caution is advised when administering to patients taking potent diuretics. Prolonged use may lead to the growth of non-susceptible organisms, necessitating regular reassessment of the patient’s condition.
