Indications
Ulticort 0.05% is a super-potent corticosteroid designed to alleviate inflammation and itching associated with corticosteroid-responsive skin conditions. It is not advisable to use this treatment for more than two consecutive weeks, and the total dosage should not exceed 50 grams per week to minimize the risk of suppressing the Hypothalamic-Pituitary-Adrenal (HPA) axis.

Pharmacology
Halobetasol propionate, like other topical corticosteroids, exhibits anti-inflammatory, antipruritic, and vasoconstrictive properties. The exact mechanism behind its anti-inflammatory effects remains unclear; however, corticosteroids are believed to stimulate the production of phospholipase A2 inhibitory proteins, known as lipocortins. These proteins may regulate the synthesis of inflammatory mediators such as prostaglandins and leukotrienes by inhibiting the release of arachidonic acid, which is derived from membrane phospholipids by phospholipase A2.
Dosage & Administration
Apply a thin layer of Halobetasol Cream or Ointment to the affected area of skin once or twice daily, as prescribed by your physician. Ensure gentle and complete application. Due to its super-potent classification, treatment should be limited to two weeks, and dosages exceeding 50 grams per week are not recommended. As with other high-potency corticosteroids, discontinue use once the desired results are achieved. If no improvement is observed within two weeks, reevaluation of the diagnosis may be necessary. Avoid using Halobetasol under occlusive dressings.

Contraindications

Side Effects
While side effects from topical corticosteroids are infrequent, they can include burning sensations, itching, dryness, folliculitis, acne, hypopigmentation, perioral dermatitis, allergic contact dermatitis, skin thinning, secondary infections, stretch marks, and miliaria.
Pregnancy & Lactation
Topical corticosteroids may only be used during pregnancy if the potential benefits outweigh the risks to the fetus. The extent of systemic absorption from topical applications and its impact on breast milk is unknown. Caution is advised when administering these corticosteroids to nursing mothers.
Precautions & Warnings
Topical corticosteroids can be systemically absorbed, potentially leading to reversible HPA axis suppression, symptoms of Cushing’s syndrome, hyperglycemia, and glucosuria. Patients using large doses of a potent topical steroid over extensive areas or under occlusive dressings should be monitored periodically for signs of HPA axis suppression through urinary free cortisol and ACTH stimulation tests. If suppression is detected, consider reducing the frequency of application, switching to a less potent steroid, or discontinuing the drug.
Use in Special Populations
Use of Ulticort cream and ointment in children under 12 years is not recommended, as safety and efficacy in this demographic have not been established. Pediatric patients are at a higher risk of HPA axis suppression when treated with topical corticosteroids.
