Indications
Traneta is prescribed for the treatment of type 2 diabetes mellitus to enhance glycemic control in adults.
- Monotherapy: For patients inadequately controlled by diet and exercise alone, and when metformin is unsuitable due to intolerance or contraindication linked to renal impairment.
- Combination therapy: In conjunction with metformin if diet, exercise, and metformin alone fail to achieve sufficient glycemic control, and alongside a sulfonylurea plus metformin when dual therapy with these medications proves insufficient.
Pharmacology
Linagliptin improves glycemic control in type 2 diabetes mellitus patients by inhibiting DPP-4 (dipeptidyl peptidase-4), an enzyme that breaks down incretin hormones like GLP-1 (glucagon-like peptide-1) and GIP (glucose-dependent insulinotropic polypeptide). By elevating active incretin hormone levels, Linagliptin stimulates insulin release from pancreatic beta cells in response to glucose and reduces glucagon secretion from pancreatic alpha cells.
Dosage & Administration
Take Linagliptin 5 mg once daily. If used with metformin, maintain the metformin dose concurrently. Combining with a sulfonylurea may necessitate a lower sulfonylurea dose to mitigate the risk of hypoglycemia.
- Renal impairment: No dosage adjustment required. Linagliptin can be taken with or without food at any time of day.
Interaction
Traneta weakly inhibits CYP3A4, with low potential for significant interactions with other medications. Clinical studies have shown Traneta has no clinically relevant effects on the pharmacokinetics of metformin, glyburide, simvastatin, warfarin, digoxin, or oral contraceptives.
Contraindications
Avoid Traneta if there is hypersensitivity to the active substance or any excipients.
Side Effects
