Tiotrop is used for the long-term management of bronchospasm in patients with chronic obstructive pulmonary disease (COPD), including conditions like chronic bronchitis and emphysema.

Pharmacology
Tiotropium is a long-acting antimuscarinic agent, often referred to as an anticholinergic. It works by selectively targeting M3 receptors in the smooth muscles of the airways, promoting bronchodilation. Tiotropium’s effect lasts over 24 hours, helping prevent bronchoconstriction and improving airflow in COPD patients. The bronchodilatory effect is primarily site-specific, acting in the airways with minimal systemic impact.
Dosage & Administration
For adults (18+), the recommended dose is 18 mcg (1 capsule) once daily via the Tiotropium Dry Powder Inhaler. The capsules should only be used with the provided inhaler device. No dosage adjustments are required for elderly patients or those with liver or kidney impairments, though close monitoring is advised in patients with moderate to severe renal issues.

Interactions
Tiotropium’s pharmacokinetics may be altered by drugs like cimetidine, which can slightly increase its concentration, or ranitidine, which has no significant effect. Close monitoring is advised when co-administering these drugs.
Contraindications
Tiotropium should not be used in patients with known hypersensitivity to atropine, ipratropium, or any components of the product.
Side Effects
Common side effects of Tiotrop include dry mouth, which usually resolves over time. Other possible anticholinergic effects include constipation, blurred vision, increased heart rate, glaucoma, urinary retention, and difficulty urinating.
Pregnancy & Lactation
Tiotropium is classified as Pregnancy Category C. It should only be used during pregnancy if the potential benefits outweigh the risks to the fetus. Its safety during labor and delivery has not been established.
Precautions & Warnings
Caution is required for patients with narrow-angle glaucoma, prostatic hyperplasia, or bladder neck obstruction, as Tiotropium may exacerbate symptoms in these conditions.
Overdose Effects
In the event of overdose, symptoms of anticholinergic toxicity may occur. No significant adverse effects have been reported with inhalation doses of up to 282 mcg in healthy individuals.
