Indications:

Telmipres is prescribed for:

• Hypertension: Management of essential hypertension in adults.
• Cardiovascular Prevention: Reduces the risk of cardiovascular events in adults with:
  • Atherothrombotic cardiovascular disease (such as coronary heart disease, stroke, or peripheral arterial disease).
  • Type 2 diabetes with documented target organ damage.

Pharmacology:

Telmisartan is an angiotensin II receptor antagonist that blocks the vasoconstrictive and aldosterone-secreting effects of angiotensin II. By binding selectively to the AT1 receptor, it prevents blood vessel constriction and aldosterone release, thus lowering blood pressure. Telmisartan does not affect ACE or bradykinin, distinguishing it from other similar medications.

Dosage & Administration:

• Initial Dose: Typically, 40 mg once daily.
• The dose may range from 20 mg to 80 mg based on individual response.
• Maximal blood pressure reduction is usually achieved within 4 weeks of treatment.
• Telmisartan can be combined with other antihypertensive medications.
• No dosage adjustment is required for elderly patients or those with renal impairment, but dialysis patients should be monitored for orthostatic hypotension.

Drug Interactions:

• NSAIDs: May increase the risk of renal impairment and reduce the effectiveness of Telmisartan.

Precautions & Warnings:

• Monitor for hypotension and adjust dosage as needed.
• Avoid use with ACE inhibitors or other angiotensin receptor blockers.
• Use caution in patients with hepatic or renal impairment.

Overdose Effects:

• Symptoms of overdose may include hypotension, dizziness, tachycardia, and possible renal failure.
