Indications:
Telmipres is prescribed for:
- Hypertension: Management of essential hypertension in adults.
- Cardiovascular Prevention: Reduces the risk of cardiovascular events in adults with:
- Atherothrombotic cardiovascular disease (such as coronary heart disease, stroke, or peripheral arterial disease).
- Type 2 diabetes with documented target organ damage.
Always follow the advice of a registered healthcare provider.
Pharmacology:
Telmisartan is an angiotensin II receptor antagonist that blocks the vasoconstrictive and aldosterone-secreting effects of angiotensin II. By binding selectively to the AT1 receptor, it prevents blood vessel constriction and aldosterone release, thus lowering blood pressure. Telmisartan does not affect ACE or bradykinin, distinguishing it from other similar medications.
Dosage & Administration:
- Initial Dose: Typically, 40 mg once daily.
- The dose may range from 20 mg to 80 mg based on individual response.
- Maximal blood pressure reduction is usually achieved within 4 weeks of treatment.
- Telmisartan can be combined with other antihypertensive medications.
- No dosage adjustment is required for elderly patients or those with renal impairment, but dialysis patients should be monitored for orthostatic hypotension.
Consult your healthcare provider for personalized dosing.
Drug Interactions:
- NSAIDs: May increase the risk of renal impairment and reduce the effectiveness of Telmisartan.
- Aliskiren: Avoid co-administration in patients with diabetes due to increased risk of adverse effects.
Contraindications:
- Known hypersensitivity to Telmisartan or any of its components.
- Do not use in patients with severe hepatic impairment or during pregnancy (especially in the second and third trimesters).
Side Effects:
- Hypertensive patients: Common side effects include sinus pain, back pain, and diarrhea.
- Cardiovascular risk patients: May experience intermittent claudication and skin ulcers.
Pregnancy & Lactation:
- Pregnancy Category C (first trimester) and Category D (second and third trimester). Telmisartan should be avoided during pregnancy due to potential fetal harm.
- Nursing mothers should decide whether to discontinue breastfeeding or the medication based on the benefits and risks.
Precautions & Warnings:
- Monitor for hypotension and adjust dosage as needed.
- Avoid use with ACE inhibitors or other angiotensin receptor blockers.
- Use caution in patients with hepatic or renal impairment.
Overdose Effects:
- Symptoms of overdose may include hypotension, dizziness, tachycardia, and possible renal failure.
Storage:
- Store Telmipres below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
