Indications
Telma Plus is a combined antihypertensive medication used to manage high blood pressure (hypertension) in adults. It can be prescribed alone or alongside other antihypertensive drugs. Lowering blood pressure with Telma Plus helps reduce the risk of serious cardiovascular events such as strokes and heart attacks.
Pharmacology
- Telmisartan: This medication inhibits the effects of angiotensin II, a key substance in the renin-angiotensin system that causes blood vessel constriction and sodium retention. By blocking angiotensin II from binding to the AT1 receptor, Telmisartan effectively lowers blood pressure without affecting other cardiovascular regulators.
- Hydrochlorothiazide: A thiazide diuretic that promotes sodium and chloride excretion, reduces plasma volume, and indirectly influences blood pressure. The combination with Telmisartan helps mitigate potassium loss typically associated with thiazide diuretics.
Dosage & Administration
- For managing Acute Bacterial Skin and Skin Structure Infections (ABSSSI):
- Intravenous: 200 mg once daily for 6 days (Infusion Time: 1 hour)
- Oral: 200 mg once daily for 6 days
- Transition between intravenous and oral forms requires no dosage adjustment. Missed doses should be taken as soon as possible within 8 hours before the next dose; otherwise, wait until the next scheduled dose.
Interactions
Caution is advised when using Telma Plus with aliskiren, digoxin, lithium, NSAIDs, corticosteroids, ACE inhibitors, and other antihypertensives. It may also affect certain laboratory tests.
Contraindications
Telma Plus is contraindicated in individuals with known hypersensitivity to Telmisartan, Hydrochlorothiazide, or any component of the medication. It should not be used with aliskiren in patients with diabetes.
Side Effects
Common side effects include dizziness, drowsiness, fatigue, flushing, back pain, nausea, diarrhea, and stomach pain. Serious adverse effects may include allergic reactions, liver enzyme elevations, leg cramps, and muscle spasms.
Pregnancy & Lactation
- Pregnancy: Category D. Use only if the benefits outweigh potential risks to the fetus. Discontinue if pregnancy occurs.
- Lactation: Use with caution, as Telmisartan and thiazides may appear in breast milk.
Precautions & Warnings
Inform your doctor of any history of kidney or liver disease, dehydration, or mineral imbalances. Monitor blood sugar levels if diabetic, and avoid potassium supplements without medical advice. Be cautious of dizziness and ensure adequate hydration.
Use in Special Populations
- Pediatric Use: Not established for children under 18.
- Geriatric Use: Caution is advised, starting with lower doses due to age-related factors.
- Hepatic Impairment: Monitor closely in patients with liver issues.
- Renal Impairment: Not recommended for severe renal impairment; mild to moderate cases do not require dose adjustments.
Overdose Effects
Symptoms of overdose may include high blood pressure, dizziness, tachycardia, bradycardia, and electrolyte imbalances. Hemodialysis is not effective for Telmisartan removal.
