Indications
Piperacillin and tazobactam, a combination product comprising a penicillin-class antibacterial and a β-lactamase inhibitor, is prescribed for treating moderate to severe infections caused by susceptible bacteria. These include:
- Intra-abdominal Infections: Such as complicated appendicitis or peritonitis caused by certain bacteria.
- Skin and Skin Structure Infections: Including cellulitis and abscesses caused by specific bacteria.
- Female Pelvic Infections: Such as postpartum endometritis or pelvic inflammatory disease.
- Community-acquired pneumonia: Of moderate severity caused by specific bacteria.
- Nosocomial pneumonia: Of moderate to severe intensity caused by specific bacteria.
Pharmacology
Piperacillin, a semi-synthetic penicillin, effectively inhibits septum and cell wall synthesis in both gram-positive and gram-negative bacteria. Tazobactam, a potent inhibitor of beta-lactamases, extends the antibiotic spectrum of piperacillin to include beta-lactamase-producing bacteria.
Dosage & Administration
Administer piperacillin and tazobactam intravenously over 30 minutes. Dosage varies based on the type and severity of infection, with adjustments for age and renal function.
Interaction
Piperacillin and tazobactam may interact with certain drugs, such as aminoglycosides, probenecid, anticoagulants, vecuronium, and methotrexate. Monitoring is crucial when co-administering these medications.
Contraindications
Avoid piperacillin and tazobactam in patients with a history of allergic reactions to penicillins, cephalosporins, or β-lactamase inhibitors.
Side Effects
Common adverse reactions may include skin rashes, gastrointestinal disturbances, fever, and injection site reactions. Serious reactions like anaphylaxis and hematological effects may occur.
Pregnancy & Lactation
Limited data exists on the use of piperacillin and tazobactam in pregnant or breastfeeding women. Caution is advised, and the benefits should outweigh the risks.
Precautions & Warnings
Monitor patients closely for serious hypersensitivity reactions, severe cutaneous adverse reactions, and hematological effects. Use with caution in critically ill patients and those with renal impairment.
Use in Special Populations
Dosage adjustments may be necessary in pediatric and geriatric patients, as well as those with renal or hepatic impairment.
Overdose Effects
Symptoms of overdose may include gastrointestinal disturbances and neuromuscular excitability. Supportive treatment is recommended.
Therapeutic Class
Piperacillin and tazobactam belong to the therapeutic class of broad-spectrum penicillins and other beta-lactam antibiotics.
Reconstitution & Storage
Reconstitute piperacillin and tazobactam powder for intravenous infusion as directed. Store at controlled room temperature, protect from light, and keep out of children’s reach.
