Indications
Sodibar serves several purposes:
- Treatment of acidosis in cardiac arrest situations.
- Alkalinization of urine.
- Management of metabolic acidosis in conditions such as severe renal disease, uncontrolled diabetes, circulatory insufficiency due to shock or dehydration, and severe primary lactic acidosis.
Pharmacology
Sodium bicarbonate acts as a systemic alkalizer, increasing plasma bicarbonate levels and buffering excess hydrogen ions, thus elevating blood pH and alleviating acidosis symptoms. Additionally, it functions as a urinary alkalizer by enhancing the excretion of free bicarbonate ions, effectively raising urinary pH. In cases of severe diarrhea with significant bicarbonate loss, it also helps replenish electrolyte imbalances.
Dosage & Administration
For oral administration, the typical dose ranges from 300 mg to 2 grams, taken 1 to 4 times daily. In severe metabolic acidosis requiring intravenous treatment, adults may receive a slow injection of a hypertonic solution up to 8.4%, or a continuous infusion of a weaker solution, usually 1.26%. During advanced cardiac life support procedures, 50 ml of an 8.4% solution may be administered. Dosage adjustments may be necessary for elderly patients.
Interactions
Sodibar may interact with various drugs including aspirin, memantine, medications with enteric coating, corticosteroids, corticotropin, alkalis, calcium, diuretics, and potassium. It may decrease the effectiveness of certain medications like ampicillin, atazanavir, ketoconazole, itraconazole, iron supplements, and sucralfate.
Contraindications
This medication is contraindicated in cases of hypersensitivity, hypoventilatory states, excessive chloride depletion, metabolic and/or respiratory alkalosis, hypocalcemia, and with diuretics known to cause hypochloremic alkalosis.
Side Effects
Common side effects may include headache, nausea, and irritability. Notify your doctor if you experience muscle weakness, slow reflexes, confusion, swelling of the feet or ankles, black tar-like stools, or coffee-ground vomit.
Pregnancy & Lactation
Use during pregnancy should be carefully considered, weighing potential benefits against risks to the fetus. It is not recommended during the second and third trimesters due to adverse renal effects observed in animal studies. Use during lactation may result in the medication passing into breast milk.
Precautions & Warnings
Exercise caution in patients with pre-existing heart, kidney, or liver disease, high blood pressure, or allergies, as this medication contains sodium. Avoid use in individuals on salt-restricted diets.
Overdose Effects
If alkalosis occurs, discontinue bicarbonate and manage according to the severity of alkalosis. Intravenous 0.9% sodium chloride injection may be administered, along with potassium chloride if hypokalemia is present. Severe alkalosis symptoms such as hyperirritability or tetany may be managed with calcium gluconate.
