Description
Indications
Seroxyn is used for the regular treatment of asthma in the following cases:
- Patients who are not adequately controlled with inhaled corticosteroids and “as needed” short-acting beta-2 agonists.
- Patients who are already adequately controlled on both inhaled corticosteroids and long-acting beta-2 agonists.
Pharmacology
- Salmeterol Xinafoate is a long-acting beta-2 agonist (12-hour duration) that helps in treating asthma by reducing bronchospasm.
- Fluticasone Propionate is a corticosteroid that works within the lungs to reduce inflammation and prevent asthma exacerbations.
Dosage
Inhaler Dosage:
- Adults and adolescents (12 years and older): Two puffs of either 25 µg salmeterol and 50 µg fluticasone, 125 µg, or 250 µg twice daily.
- Children (4-12 years): Two puffs of 25 µg salmeterol and 50 µg fluticasone twice daily.
Inhalation Capsule (Asthma):
- Adults and Adolescents (12 years and older): Salmeterol 50 µg & Fluticasone 100 µg or 250 µg twice daily.
- Children (4-11 years): Salmeterol 50 µg & Fluticasone 100 µg twice daily.
Maxhaler (For Asthma):
- Adults and Adolescents (12 years and older): 50/100, 50/250, or 50/500 inhaler once or twice daily depending on severity.
Maxhaler (For COPD):
- Adults and Adolescents (12 years and older): One inhalation twice daily.
Administration
- Shake the inhaler well before each use.
- Exhale fully before inhaling the medication.
- Hold the inhaler upright, place the mouthpiece between your teeth, and inhale slowly while pressing the inhaler to release the dose.
- Hold your breath for 10 seconds after inhaling.
- Rinse your mouth after each use to reduce side effects like thrush.
Interactions
- Avoid using non-selective beta-blockers in asthma patients, unless absolutely necessary.
- Be cautious when using strong CYP3A4 inhibitors (e.g., ketoconazole) as they may increase Fluticasone Propionate levels.
Contraindications
- Hypersensitivity to Salmeterol or Fluticasone or any excipients.
- Not recommended for acute asthma attacks.
Side Effects
- Salmeterol: Tremors, headaches, palpitations, arrhythmias, and muscle cramps.
- Fluticasone: Hoarseness, candidiasis (thrush), cutaneous hypersensitivity, and rare facial or oropharyngeal edema.
Pregnancy and Lactation
- Pregnancy: Use only if the benefits outweigh the risks.
- Lactation: Use with caution, as the amounts in breast milk are likely low.
Precautions and Warnings
- Asthma-related adverse events: Seek medical advice if asthma symptoms worsen.
- Paradoxical Bronchospasm: Can occur with inhalation therapy; treat immediately with a fast-acting bronchodilator.
- COPD patients: Watch for pneumonia, as it may occur more frequently in these patients.
Overdose Effects
- Salmeterol Overdose: Symptoms include seizures, tachycardia, arrhythmias, nervousness, and insomnia.
- Fluticasone Overdose: May cause temporary adrenal suppression.
