Seroxyn 50/250 Acucap

Indications

Seroxyn is recommended for the regular management of asthma in patients where combination therapy (long-acting β2-agonist and inhaled corticosteroid) is suitable. Specifically, it is used in:

• Patients not adequately controlled with inhaled corticosteroids and “as-needed” short-acting β2-agonists.
• Patients already controlled with both inhaled corticosteroids and long-acting β2-agonists.

Pharmacology

Seroxyn combines two active ingredients: Salmeterol Xinafoate and Fluticasone Propionate. These components work synergistically to manage asthma and other airway obstructions:

• Salmeterol Xinafoate is a selective, long-acting beta-2 agonist that provides protection against asthma symptoms for up to 12 hours.
• Fluticasone Propionate is a corticosteroid with potent anti-inflammatory effects. When inhaled, it targets the lungs to reduce asthma symptoms and prevent exacerbations without causing systemic side effects at usual doses.

Dosage

Inhaler (for Asthma):

• Adults & Adolescents (12 years and older): 2 puffs of Salmeterol 25 µg and Fluticasone Propionate 50 µg, 125 µg, or 250 µg twice daily.
• Children (4-12 years): 2 puffs of Salmeterol 25 µg and Fluticasone Propionate 50 µg twice daily.

Inhalation Capsule (for Asthma):

• Adults & Adolescents (12 years and older): 50 µg Salmeterol & 100 µg Fluticasone, or 50 µg Salmeterol & 250 µg Fluticasone, twice daily.
• Children (4-11 years): Salmeterol 50 µg & Fluticasone 100 µg twice daily.

Maxhaler (For Asthma):

• Adults & Adolescents (12 years and older): One inhalation of either 50/100, 50/250, or 50/500 inhaler twice daily.
• Children (4 years and older): One inhalation of 50/100 inhaler twice daily.

Maxhaler (For COPD):

• One inhalation twice daily.

Administration

Proper inhaler technique is crucial for effective treatment. Follow these steps:

1. Shake the inhaler well (6 times).
2. Exhale fully, place the inhaler between your teeth, and close your lips around the mouthpiece.
3. Inhale deeply and slowly while pressing the canister to release the medication.
4. Hold your breath for 10 seconds, then exhale slowly.
5. Wait 1 minute between puffs if prescribed multiple inhalations.
6. After use, rinse your mouth with water to prevent side effects like hoarseness or oral thrush.

Clean your inhaler weekly to maintain effectiveness.

Interaction

Be cautious when using Seroxyn with certain medications, particularly β-blockers or CYP3A4 inhibitors (e.g., ketoconazole, ritonavir), as they may increase the effects of Fluticasone Propionate.

Contraindications

Seroxyn is contraindicated in patients with a history of hypersensitivity to Salmeterol Xinafoate, Fluticasone Propionate, or any of the excipients. It should not be used for acute asthma attacks or in cases of severe asthma.

Side Effects

Possible side effects include:

• Salmeterol: Tremors, palpitations, headache, cardiac arrhythmia, oropharyngeal irritation, and muscle cramps.
• Fluticasone Propionate: Hoarseness, oral thrush, cutaneous hypersensitivity, and facial edema.

Both components are generally well-tolerated, but any new or worsening symptoms should be reported to a healthcare provider.

Pregnancy & Lactation

• Pregnancy: Use during pregnancy should only be considered if the benefit to the mother outweighs the potential risk to the fetus.
• Lactation: Both Salmeterol and Fluticasone Propionate are excreted in breast milk in low concentrations. Consult a healthcare provider before use during breastfeeding.

Precautions & Warnings

• Not recommended during acute asthma exacerbations.
• Be vigilant for asthma-related side effects such as worsening symptoms or paradoxical bronchospasm.
• Regularly review the treatment plan to adjust the dose to the minimum effective dose.
• Special care should be taken with COPD patients due to the increased risk of pneumonia.

Special Populations

• Pediatric patients may experience reduced growth velocity with long-term use of inhaled corticosteroids. Regular monitoring is recommended.
• No dosage adjustment is necessary for elderly patients or those with renal impairment, but use in hepatic impairment is not well-studied.

Overdose Effects

• Salmeterol Overdose: Symptoms include seizures, hypertension, tachycardia, tremor, and palpitations. Treat with cardioselective beta-blockers with caution.
• Fluticasone Propionate Overdose: Chronic overdose can lead to adrenal suppression, requiring monitoring.

Storage Conditions

Store below 30°C, away from direct sunlight and heat. Keep the canister tightly closed and out of reach of children. Do not puncture or incinerate the canister, even if it appears empty.
