Rosuvastatin serves as an effective treatment for various conditions, including:
- Heterozygous and Homozygous Hypercholesterolemia (Familial and Non-familial)
- Mixed Dyslipidemia (Fredrickson Type IIa and IIb)
- Primary prevention of cardiovascular disease
Pharmacological Action
Rosuvastatin acts as a selective and competitive inhibitor of HMG-CoA reductase, the pivotal enzyme responsible for converting 3-hydroxy-3-methyl glutaryl coenzyme A to mevalonate, a precursor of cholesterol. Its lipid-modifying effects occur through two mechanisms:
- Enhancement of hepatic LDL receptors, promoting the uptake and breakdown of LDL.
- Inhibition of hepatic synthesis of VLDL, thereby reducing the total number of VLDL and LDL particles.
Dosage Guidelines
- Recommended daily dose: 5-40 mg, with a 40 mg dosage only for patients failing to achieve LDL-C goals with a 20 mg dose.
- For HoFH: Initiate with 20 mg/day.
- Pediatric Dosage:
- For HeFH: 5-10 mg/day (age 8 to less than 10 years), 5-20 mg/day (age 10 to 17 years).
- For HoFH: 20 mg/day (age 7 to 17 years).
Administration
Rosuvastatin can be administered with or without food at any time of the day, as per medical advice.
Drug Interactions
Notable drug interactions include:
- Cyclosporine: Limit Rosuvastatin dose to 5 mg once daily when co-administered.
- Gemfibrozil: Avoid combination or limit Rosuvastatin dose to 10 mg once daily.
- Lopinavir/Ritonavir or atazanavir/ritonavir: Reduce Rosuvastatin dose to 10 mg once daily.
- Coumarin anticoagulants: Monitor INR closely and achieve stability before initiating Rosuvastatin therapy.
Contraindications
