Indications
Robic serves as a reliable treatment option for various conditions including Amoebiasis (both intestinal and hepatic manifestations), Giardiasis, Trichomoniasis, Bacterial vaginosis, and for combating susceptible anaerobic infections.
Pharmacology
Derived from the 5-nitroimidazole family, Ornidazole displays robust activity against protozoa and anaerobic bacteria. Through biochemical conversion, it targets DNA, disrupting its helical structure and impeding protein synthesis, ultimately leading to cell death in susceptible organisms.
Dosage & Administration
For Amoebiasis, adults typically take 500 mg twice daily for 5 days, while children’s dosage ranges from 10-25 mg per kg body weight divided into two doses.
In cases of Amoebic dysentery, adults are prescribed 1.5 gm once daily for 3 days, while children’s dosage is 40 mg per kg body weight, once a day for 3 days.
For Giardiasis, adults usually take 1.5 gm once daily for 1-2 days, while children’s dosage is 40 mg per kg body weight for 2 days.
Treatment for Trichomoniasis involves a dosage of 1.5 gm once daily or 500 mg twice daily for 5 days. It’s essential for the sexual partner to undergo simultaneous treatment.
Bacterial vaginosis can be addressed with either 3 tablets of 500 mg each as a single dose or one tablet of 500 mg once daily for 5-7 days, as advised by a healthcare professional.
Interaction
While Robic bears a mild potential for disulfiram-like reactions similar to other imidazoles, caution is advised when administered concurrently with oral anticoagulants due to potential increased risk of hemorrhage. Additionally, Robic may decrease the clearance of 5-fluorouracil.
Contraindications
Prior hypersensitivity to Ornidazole or other nitroimidazoles renders it contraindicated. Moreover, individuals with central nervous system disorders, especially epilepsy or peripheral neuropathy, should avoid its usage.
Side Effects
Side effects primarily manifest in the gastrointestinal tract (nausea, vomiting, epigastric pain) and the central nervous system (dizziness, headache, lassitude). Unlike some nitroimidazoles, Robic does not exhibit interactions with alcohol, although further research is warranted.
Pregnancy & Lactation
Clinical trials regarding its safety during pregnancy and lactation are insufficient. Ornidazole should only be prescribed if the potential benefits outweigh the risks to the fetus or neonate.
Precautions & Warnings
Patients presenting with ataxia, vertigo, or mental confusion should receive Robic cautiously. Rarely, prolonged usage may lead to mild leukopenia, necessitating a reassessment of therapy.
Use in Special Populations
In patients with renal impairment undergoing hemodialysis, a supplemental dose (50% of the usual dose) prior to dialysis is recommended. Similarly, in cases of severe hepatic impairment, doubling the interval between doses is advisable.
