Indications
Revira is indicated for treating infections caused by the Varicella zoster virus (VZV) in immunocompetent adults, including herpes zoster (shingles) and ophthalmic zoster. It is also effective in treating herpes zoster in adult patients with mild to moderate immunosuppression. Revira is further used for managing infections caused by the Herpes simplex virus (HSV), including genital herpes and recurrent ocular HSV infections.
Pharmacology
Revira, the hydrochloride salt of valacyclovir, an antiviral drug, inhibits the growth and spread of herpes viruses. By slowing viral replication, it helps the body combat infections, reducing symptoms and duration of illness. Valacyclovir is prescribed for genital herpes, shingles, and cold sores.
Dosage & Administration
For herpes zoster (shingles) caused by VZV in immunocompetent adults, the recommended dose is Valacyclovir 1000mg three times daily for 7 days. Dosage may vary for immunocompromised patients.
For HSV infections, dosing depends on the severity and recurrence:
- Initial genital herpes episodes: Valacyclovir 1000mg twice daily for 7-10 days.
- Recurrent genital herpes: Valacyclovir 500mg twice daily for 3-5 days.
- Suppression therapy: Valacyclovir 500mg once daily or as directed by a healthcare provider.
Interaction
Revira may interact with nephrotoxic drugs and requires careful monitoring, especially in patients with renal impairment. Drugs like cimetidine and probenecid can increase Revira levels in the body.
Contraindications
Revira should not be used in individuals with hypersensitivity to Valacyclovir, aciclovir, or any excipients.
Side Effects
Common side effects include headache and nausea. Serious side effects may include neurological symptoms and renal impairment, particularly in elderly patients or those with existing kidney issues.
Pregnancy & Lactation
Use of Valacyclovir during pregnancy should be based on risk-benefit assessment, as data on its safety in pregnant women are limited. It is excreted in breast milk, but at therapeutic doses, effects on breastfed infants are minimal.
Precautions & Warnings
Special precautions are needed in patients with renal impairment and elderly patients due to the potential for increased side effects. Careful monitoring and dosage adjustments are essential.
Overdose Effects
Overdose of Revira may lead to acute renal failure and neurological symptoms. Management includes supportive care and, if necessary, hemodialysis to remove excess drug from the bloodstream.
