Description

Indications

Reset is used for the relief of fever, common cold, and flu symptoms, as well as for treating headaches, toothaches, earaches, body aches, myalgia, neuralgia, dysmenorrhea, sprains, colic pain, back pain, postoperative pain, postpartum pain, inflammatory pain, and pain following vaccinations in children. It is also effective for alleviating rheumatic and osteoarthritic pain, as well as joint stiffness.

Pharmacology

Paracetamol, a widely used analgesic and antipyretic, has mild anti-inflammatory properties. It is believed to work primarily in the central nervous system (CNS) by raising the pain threshold through the inhibition of cyclooxygenase (COX-1, COX-2, and COX-3), enzymes involved in prostaglandin (PG) synthesis. Known for its fast-acting and safe profile, Paracetamol is well tolerated and avoids the side effects commonly associated with aspirin.

Dosage & Administration

Tablets:

  • Adults: 1-2 tablets every 4-6 hours, up to a maximum of 4g (8 tablets) per day.
  • Children (6-12 years): ½ to 1 tablet, 3-4 times daily. For long-term use, do not exceed 2.6g/day.

Extended Release Tablets:

  • Adults & Children (12+ years): 2 tablets, taken whole, every 6-8 hours (up to 6 tablets in 24 hours). Do not crush the tablet.

Syrup/Suspension:

  • Infants (<3 months): 10 mg/kg body weight (5 mg/kg for jaundiced infants), 3-4 times daily.
  • 3 months to 1 year: ½ to 1 teaspoonful, 3-4 times daily.
  • 1-5 years: 1-2 teaspoonfuls, 3-4 times daily.
  • 6-12 years: 2-4 teaspoonfuls, 3-4 times daily.
  • Adults: 4-8 teaspoonfuls, 3-4 times daily.

Suppositories:

  • Children (3-12 months): 60-120 mg, 4 times daily.
  • Children (1-5 years): 125-250 mg, 4 times daily.
  • Children (6-12 years): 250-500 mg, 4 times daily.
  • Adults & children (12+ years): 500-1000 mg, 4 times daily.

Paediatric Drops:

  • Infants (up to 3 months): 0.5 ml (40 mg).
  • 4-11 months: 1.0 ml (80 mg).
  • 1-2 years: 1.5 ml (120 mg). Do not exceed 5 doses per day, for a maximum of 5 days.

Paracetamol Tablets with Actizorb Technology:
These tablets dissolve up to five times faster than regular Paracetamol tablets, offering quick relief for pain and fever. They are ideal for patients who cannot tolerate aspirin or other analgesics.

  • Adults & Children (12+ years): 1-2 tablets every 4-6 hours as needed (maximum 8 tablets in 24 hours).
  • Children (7-11 years): ½-1 tablet every 4-6 hours as needed (maximum 4 tablets in 24 hours). Not recommended for children under 7 years.

Interactions

Reset may have reduced efficacy in patients who have used barbiturates, tricyclic antidepressants, or consumed alcohol. Alcohol increases the risk of liver toxicity from an overdose of Reset. Chronic use of anticonvulsants or oral contraceptives may accelerate the metabolism of Reset, reducing its effectiveness.

Contraindications

Reset should not be used by individuals with a known hypersensitivity to Paracetamol.

Side Effects

Side effects are typically mild but can include hematological issues like thrombocytopenia, leucopenia, pancytopenia, neutropenia, and agranulocytosis. Other rare side effects include pancreatitis, skin rashes, and allergic reactions.

Pregnancy & Lactation

Paracetamol is generally considered safe during pregnancy when used at recommended doses. However, it is advisable to consult with a doctor before using it. Paracetamol is excreted in breast milk in negligible amounts, and breastfeeding is not contraindicated.

Precautions & Warnings

Caution is recommended when administering Reset to patients with severe liver or kidney impairment. The risk of overdose is higher in individuals with non-cirrhotic alcoholic liver disease. Do not exceed the recommended dose and avoid simultaneous use of multiple Reset-containing products. This medication should be taken only under the supervision of a healthcare provider.

Overdose Effects

An overdose of Reset can lead to liver damage, especially in individuals who ingest more than 10g. People on long-term medications such as Carbamazepine, Phenobarbitone, or Phenytoin, or those who consume alcohol regularly, are at higher risk. Symptoms of overdose include pallor, nausea, vomiting, anorexia, and abdominal pain. Liver damage may become noticeable 12-48 hours after ingestion. In severe cases, the condition can progress to hepatic failure, encephalopathy, hypoglycemia, and even death.

Immediate medical attention is necessary for Reset overdose. If taken within 1 hour, activated charcoal may be administered, and N-acetylcysteine can be used up to 24 hours post-ingestion, with maximum effectiveness achieved within 8 hours.
