Description

Indications:

Remus ointment serves as a therapeutic option for patients with moderate to severe atopic dermatitis, especially when conventional therapies pose risks or show inadequate responsiveness. It also finds utility in various other dermatological conditions such as chronic cutaneous graft-vs-host disease, hand and foot eczema, allergic contact dermatitis, vitiligo, psoriasis, and more.

Pharmacology:

Tacrolimus, the active ingredient in Remus ointment, is a macrocyclic immunomodulator derived from the fungus Streptomyces tsukubaensis. By inhibiting T-lymphocyte activation, Tacrolimus disrupts the inflammatory cascade, offering relief from dermatological symptoms.

Dosage & Administration:

For adults, apply a thin layer of Tacrolimus ointment twice daily to affected areas, continuing treatment for a week post-symptom resolution. Pediatric dosages vary: 0.03% ointment for children aged 2 years and older, and 0.1% ointment for those 16 years and older. Caution is advised against occlusive dressings.

Interaction:

Although formal studies on drug interactions are lacking, caution is warranted when administering Tacrolimus ointment alongside known CYP3A4 inhibitors like erythromycin or ketoconazole.

Contraindications:

Tacrolimus ointment is contraindicated in individuals with hypersensitivity to Tacrolimus or any component of the formulation.

Side Effects:

Rare side effects of Remus ointment may include skin burning, pruritus, flu-like symptoms, allergic reactions, erythema, infections, and headaches.

Pregnancy & Lactation:

Considered Pregnancy Category C, Tacrolimus should be cautiously used during pregnancy and lactation due to its excretion in human milk and potential for adverse effects in nursing infants.

Precautions & Warnings:

Exercise caution in patients predisposed to superficial skin infections. The safety of Remus ointment is not established in cases of generalized erythroderma.
