Pharmacology
The key action of glyceryl trinitrate is through the release of nitric oxide, inducing relaxation of the internal anal sphincter, a key factor in anal fissure formation.
Dosage & Administration
Nitroglycerin 0.4% ointment should be gently inserted into the anal cavity using an applicator, at least 1 cm into the anus. It’s recommended to apply the ointment intra-anally every twelve hours until pain subsides, up to a maximum of 8 weeks.
Interactions
Several medications, including those for depression, erectile dysfunction, high blood pressure, diuretics, and others, may enhance the blood pressure-lowering effects of glyceryl trinitrate ointment.

Overdose Effects
Accidental overdose may lead to hypotension and reflex tachycardia. Prudent management involves increasing central fluid volume to counteract hypotension.
