Description

Indications

Pharmacology

Hydroxychloroquine Sulphate is a synthetic derivative of quinine that functions as an immunosuppressant. It reduces the production of rheumatoid and acute-phase reactants, accumulates in white blood cells, stabilizes lysosomal membranes, and inhibits enzymes such as collagenase and proteases that contribute to cartilage degradation. Additionally, it disrupts the cell walls of infected red blood cells, effectively eliminating developing parasites.

Dosage & Administration

Hydroxychloroquine Sulphate tablets should be taken orally with food to minimize gastrointestinal discomfort.

  • Acute and Chronic Rheumatoid Arthritis: 400 to 600 mg daily. Once a favorable response is observed (typically within 4 to 8 weeks), the dose can be reduced by 50%.
  • Systemic Lupus Erythematosus (SLE): 400 mg once or twice daily for several weeks or months, depending on the patient’s response. The maintenance dose is 200 to 400 mg daily.
  • Malaria:
    • Adults: Start with 800 mg, followed by 400 mg after 6-8 hours, and then 400 mg daily for two consecutive days.
    • Children: A total dose of 25 mg/kg over 3 days, divided as follows:
      • First dose: 10 mg/kg (not exceeding 620 mg).
      • Second dose: 5 mg/kg (not exceeding 310 mg) 6 hours after the first.
      • Third dose: 5 mg/kg 18 hours after the second.
      • Fourth dose: 5 mg/kg 24 hours after the third.

Side Effects

Reconil is typically well tolerated; however, some users may experience mild side effects such as nausea, vomiting, stomach upset, loss of appetite, diarrhea, fatigue, weakness, headache, or visual disturbances during the initial days of treatment.

Pregnancy & Lactation

This medication should only be used during pregnancy when absolutely necessary. Since small amounts can pass into breast milk, consult your doctor prior to use if breastfeeding.

Precautions & Warnings

Children are particularly sensitive to 4-aminoquinoline compounds. It is essential to keep these medications out of children’s reach. Regular ophthalmologic examinations are recommended every 12 months.

Overdose Effects

Overdose symptoms may include headache, drowsiness, visual disturbances, cardiovascular collapse, and convulsions, potentially leading to respiratory and cardiac arrest. Gastric lavage should be performed until the stomach is fully emptied.
