Indications
Perinase is used for:
- Preventing and treating seasonal allergic rhinitis, including hay fever, and perennial rhinitis.
- Exhibiting strong anti-inflammatory effects on the nasal mucosa with minimal systemic absorption.
Description
Perinase is a synthetic trifluroinated corticosteroid known for its anti-inflammatory properties. It comes as an aqueous suspension in a metered-dose nasal spray, delivering 120 precise sprays per bottle. This formulation is designed for topical application to the nasal mucosa.
Pharmacology
When applied topically, Fluticasone Propionate, the active ingredient in Perinase, offers anti-inflammatory and vasoconstrictor benefits. Although the exact mechanisms are not fully understood, it may reduce:
- The presence of mediator cells like basophils, leukocytes, and mast cells in the nasal epithelium.
- Eosinophil counts and the sensitivity of sensory nerves.
- Secretory responses to cholinergic stimulation and fibroblast activity. Other potential effects include decreased capillary dilation, stabilized lysosomal membranes, and inhibition of proteolytic enzyme release.
Dosage
For Adults and Children Aged 12 and Over:
- Administer 2 sprays in each nostril once daily, preferably in the morning. For some individuals, 2 sprays in each nostril twice daily may be necessary, not exceeding 4 sprays in total.
For Children Aged 4-11 Years:
- Administer 1 spray in each nostril once daily, with a maximum of 4 sprays per day. Usage for perennial rhinitis in children under 12 is not recommended due to insufficient data.
For Children Under 4 Years:
- The safety and efficacy of Perinase nasal spray in children under 4 have not been established.
Patients should adhere to the prescribed dosage schedule for optimal effectiveness. Always follow your physician’s instructions.
Administration
How to Use the Nasal Spray:
- Shake the bottle gently and remove the dust cover.
- Hold the bottle with your forefinger and middle finger on either side of the nozzle and your thumb underneath. Press down until a fine mist appears. If this is your first use or it has been a week since the last use, pump several times until a mist emerges.
- Clear your nostrils by gently blowing your nose.
- Close one nostril and insert the applicator into the open nostril. Tilt your head slightly forward and keep the spray upright. Inhale through your nose while pressing the applicator firmly to release the spray.
- Exhale through your mouth.
- Repeat the process for the other nostril if necessary.
Cleaning the Nasal Spray:
- Remove the dust cover.
- Detach the nasal applicator.
- Wash both the applicator and dust cover in warm water.
- Shake off excess water and let them air dry. Avoid using additional heat.
- Reattach the applicator and dust cover.
Contraindications
Perinase nasal spray is not suitable for individuals with hypersensitivity to any of its components.
Side Effects
Common side effects include nasal and throat dryness, an unpleasant taste or smell, and occasional nosebleeds. Systemic absorption is minimal, reducing the likelihood of widespread side effects.
Pregnancy and Lactation
Adequate studies in pregnant women are lacking. Perinase should only be used during pregnancy if the benefits outweigh potential risks. It is unknown whether Fluticasone Propionate is excreted in breast milk, so caution is advised when used by nursing mothers.
Precautions and Warnings
Rare cases of wheezing, nasal septum perforation, cataracts, glaucoma, and increased intraocular pressure have been reported. Systemic effects are minimal at recommended doses, but higher doses may increase risks. Avoid exceeding the recommended dosage.
