Treatment Applications
For managing mild to moderately severe bacterial infections caused by penicillin-susceptible pathogens responsive to oral penicillin therapy, Live Pharmacy provides solutions for various conditions including:
- Infections affecting the ear, nose, and throat regions such as tonsillitis, pharyngitis, laryngitis, otitis media, and sinusitis.
- Lower respiratory tract infections including bronchitis, pneumonia, and bronchopneumonia.
- Infections caused by beta-hemolytic streptococci of group A, such as scarlet fever, erysipelas, and rheumatic fever.
- Skin infections like pyoderma, furunculosis, phlegmon, erysipeloid, and erythema migrans, assuming the microorganisms are susceptible to penicillin.
- Bacterial lymphadenitis and lymphangitis.
- Buccal cavity, gum, or jaw infections, including inflammatory infiltrates, delayed dentition stages II and III, antral fistulas, and secondary bacterial infections post virus-induced gingivitis or stomatitis.
- Scarlet fever prophylaxis and prevention of rheumatic fever recurrences.
- Prophylactic measures post-dental and oral surgical procedures, particularly for high-risk patients with congenital cardiac defects, artificial heart valves, or rheumatic endocarditis. Combining with another suitable antibiotic might be necessary in some cases.
Pharmacological Details
Phenoxymethyl penicillin, also known as penicillin V, exhibits acid stability and gets absorbed from the upper part of the small intestine. Among its forms, the potassium salt of Phenoxymethyl penicillin offers optimal absorption. While it can be taken with meals, the highest absorption occurs when administered orally at least one hour before or two hours after eating. This medication presents a convenient option for treating Gram-positive infections and has the advantage of being resistant to gastric acid inactivation compared to penicillin G.
Dosage Recommendations
The dosage of Phenoxymethyl penicillin should be based on the sensitivity of the causative microorganism, the severity of the infection, and the patient’s clinical response.
- Adults: 250-500 mg every six hours.
- Children (above 1 year): 125-250 mg every six hours.
- Suspension powder options:
- 125 mg/5 ml: 1-2 teaspoonfuls (5-10 ml) every six hours.
- 250 mg/5 ml: ½-1 teaspoonful (2.5-5 ml) every six hours.
- Suspension powder options:
- Infants (below 1 year): 62.5-125 mg every six hours.
- Suspension powder option: 125 mg/5 ml: ½-1 teaspoonful (2.5-5 ml) every six hours or as prescribed by the physician.
Phenoxymethyl penicillin is most effective when taken on an empty stomach, preferably at least one hour before or two hours after a meal.
Administration Instructions
Phenoxymethyl Penicillin is optimally taken on an empty stomach, approximately one hour before meals. Tablets should be swallowed whole with ample liquid. Shake the syrup vigorously before each use.
Interactions
Food: Taking Penvik DS with food reduces the absorption rate. To maximize absorption, it’s best taken on an empty stomach, ideally one hour before meals.
Drug interactions: Combining penicillins with other medications like methotrexate may increase serum levels and enhance toxic effects, necessitating monitoring of methotrexate levels. Diarrhea resulting from Penvik DS treatment may affect the absorption of other orally administered drugs, potentially impairing efficacy. Concurrent use with certain medications like probenecid, indomethacin, phenylbutazone, salicylates, sulfinpyrazone, or oral anticoagulants may have varying effects on serum levels of phenoxymethylpenicillin.
Contraindications
Phenoxymethyl Penicillin should not be given to patients with penicillin hypersensitivity or severe gastrointestinal disorders accompanied by vomiting and diarrhea.
Side Effects
Potential side effects of Phenoxymethyl Penicillin may include skin hypersensitivity reactions, eosinophilia, or more severe allergic reactions like drug fever, vasculitis, serum sickness, or interstitial nephritis. In rare instances, anaphylactic or anaphylactoid reactions may occur. Other possible adverse effects include gastrointestinal disturbances, pseudomembranous colitis, and changes in blood picture such as leukopenia, agranulocytosis, or myelosuppression.
Pregnancy & Lactation
Phenoxymethylpenicillin can cross the placenta and pass into breast milk in small amounts. It may be used during pregnancy and lactation under appropriate indications, although caution is advised due to potential infant side effects like diarrhea and yeast colonization of mucous membranes.
Precautions & Warnings
Consideration should be given to the possibility of cross-allergy between cephalosporins and penicillins. In patients with heart diseases or electrolyte disturbances, the potassium content of Penvik DS may need consideration. Patients taking beta-lactams should be monitored for encephalopathy risks, especially in cases of overdose or renal impairment.
Overdose Effects
Phenoxymethylpenicillin has low toxicity and a broad therapeutic range. However, overdose, particularly in cases of renal impairment, may increase the risk of encephalopathy. No special measures are typically required beyond discontinuing the medication, and elimination can occur through hemodialysis if necessary.
Therapeutic Class & Storage
Phenoxymethylpenicillin belongs to the therapeutic class of Benzylpenicillin & Phenoxymethyl penicillin. Store it in a cool, dry place away from light for optimal preservation.
