Indications
Constipation (Chronic Constipation): Osmolax is indicated when a fiber-rich diet, adequate liquids, and physical exercise are insufficient.
Intestinal Flora Disturbances: Effective for conditions such as damaged intestinal flora (post-antibiotic treatment), gall bladder diseases, and intestinal diseases (e.g., colitis, diverticulosis, megacolon).
Increased Blood Ammonia Levels: Used to treat hyperammonemia in hepatopathy and portal-systemic encephalopathy.
Description
Osmolax is a hyperosmolar laxative, effective for treating constipation. It is a non-absorbable disaccharide that draws water into the bowel, promoting soft stool formation and accelerating bowel motion.
Pharmacology
Lactulose, the active ingredient in Osmolax, is a synthetic disaccharide. In the colon, it is metabolized by saccharolytic bacteria into organic acids, which lower colon pH, retain water osmotically, and increase peristaltic activity.
Dosage & Administration
Chronic Constipation:
- Adults: Initially 3-6 teaspoons daily; long-term therapy: 1½-6 teaspoons daily.
- Children (up to 14 years): Initially 3 teaspoons daily; long-term therapy: 1-2 teaspoons daily.
- Infants and Toddlers: Initially 1-2 teaspoons daily; long-term therapy: 1 teaspoon daily.
Damaged Intestinal Flora:
- Adults: 1-2 teaspoons daily.
- Children: 1 teaspoon daily.
Reduction of Blood Ammonia Levels:
- Hyperammonemia in hepatopathy: Maximum 18-30 teaspoons daily.
- Portal systemic encephalopathy: Hourly doses of 6-9 teaspoons until the desired laxative effect is achieved, then reduced.
Interaction
No significant drug interactions reported. Cardiac glycoside effects can be intensified by potassium deficiency in cases of abuse.
Contraindications
Hypersensitivity to galactose or lactose, galactose-free diet, gastro-cardial symptom complex, and suspected intestinal obstruction.
Side Effects
Possible side effects include flatulence, cramping, and abdominal discomfort, especially at the start of treatment. Overdose can cause diarrhea and electrolyte imbalance.
Pregnancy & Lactation
FDA Pregnancy Category B: Studies show no adverse effects of Lactulose. Use during pregnancy and lactation should be decided by a physician.
Precautions & Warnings
Administer with caution to patients intolerant to Osmolax. High doses used for (pre) coma hepaticum should be monitored in diabetics.
Overdose Effects
Acute overdosage is rare but may cause diarrhea and abdominal cramps. Chronic misuse may result in hypercortisolism; discontinue use if symptoms occur.
