Pharmacology
Osartil Plus combines Losartan and Hydrochlorothiazide. Losartan, an angiotensin II receptor blocker, inhibits the vasoconstrictor effects of angiotensin II, reducing blood pressure and preventing aldosterone secretion. Hydrochlorothiazide is a thiazide diuretic that increases sodium and chloride excretion, reducing plasma volume and indirectly enhancing the effectiveness of Losartan. This combination effectively manages hypertension while addressing electrolyte balance.
Dosage
Hypertension:
- The usual starting dose is one Osartil Plus tablet (50/12.5 mg) taken once daily.
- If blood pressure is not adequately controlled, the dosage may be increased to one tablet (100/25 mg) once daily.
- For hypertensive patients with left ventricular hypertrophy, the initial dose is 50/12.5 mg, which can be adjusted as needed, with a maximum of 100/25 mg once daily.
- The antihypertensive effect typically manifests within three weeks.
Severe Hypertension:
- The starting dose for severe hypertension is one tablet (50/12.5 mg) daily, which can be increased to 100/25 mg once daily if necessary.

Interaction
Losartan exhibits minimal pharmacokinetic interactions with Hydrochlorothiazide, Digoxin, Warfarin, and others. However, co-administration with potassium-sparing diuretics or supplements may increase serum potassium levels. The antihypertensive effect can be reduced by non-steroidal anti-inflammatory drugs like indomethacin.
Contraindications
Osartil Plus is contraindicated in individuals with hypersensitivity to any components or with anuria. It should also be avoided in patients with hypersensitivity to sulfonamide-derived drugs.
Side Effects
Common side effects are generally mild and may include dizziness, symptomatic hypotension, and hyperkalemia. Rarely, serious effects such as angioedema, gastrointestinal disturbances, and renal impairment may occur.
Pregnancy & Lactation
Angiotensin II receptor antagonists like Osartil Plus should be avoided during pregnancy due to potential adverse effects on fetal and neonatal health. Limited information is available regarding breastfeeding; alternatives with established safety profiles are recommended.
Precautions & Warnings
- Monitor for hypersensitivity reactions and electrolyte imbalances.
- Caution is advised in patients with renal impairment or symptomatic hypotension.
Use in Special Populations
- Renal Impairment: Dosage can follow the usual regimen if creatinine clearance is >30 ml/min. Loop diuretics are preferred for severe renal impairment.
- Hepatic Impairment: Not recommended for titration due to dosage limitations.
- Pediatric Patients: Safety and efficacy in children have not been established.
Overdose Effects
Overdose may result in hypotension and tachycardia. Supportive treatment is recommended. Neither Losartan nor Hydrochlorothiazide is effectively removed by hemodialysis.
