Indications
- Asthma Management: Onriva is designed to manage airflow obstruction in patients aged 12 and older. It is not intended for the immediate relief of acute bronchospasm.
- Chronic Obstructive Pulmonary Disease (COPD): Effective for long-term management of airflow obstruction in COPD, including chronic bronchitis and emphysema.
Description
Onriva is a long-acting β2-adrenergic agonist used for once-daily maintenance treatment. It combines Formoterol Fumarate Dihydrate and Budesonide. Formoterol provides rapid, long-lasting bronchial relaxation, while Budesonide offers anti-inflammatory benefits, reducing asthma symptoms and exacerbations.
Pharmacology
- Budesonide: A potent corticosteroid with strong anti-inflammatory effects, targeting multiple inflammatory cells and mediators.
- Formoterol Fumarate Dihydrate: A β2-adrenergic agonist that promotes bronchial smooth muscle relaxation by increasing cyclic AMP levels.
Dosage & Administration
- For Asthma:
- Adults & Adolescents (12+): 1-2 capsules twice daily; up to 12 capsules per day if needed.
- Children (4+): 1 capsule once daily; up to 8 capsules per day if required.
- For COPD:
- Adults (40+): 200 mcg capsules twice daily (max 4/day) or 400 mcg capsules twice daily (max 2/day).
Administration
Inhalation capsules should not be swallowed. Use only with the provided inhalation device. Administer once daily at the same time. Clean the inhaler weekly and check technique regularly.
Interactions
Avoid with strong cytochrome P450 3A4 inhibitors. Use cautiously with MAO inhibitors, tricyclic antidepressants, beta-blockers, and diuretics.
Contraindications
Not for asthma without a long-term control medication. Contraindicated in patients with hypersensitivity to Onriva or its ingredients.
Side Effects
- Common: Cough, nasopharyngitis, headache, nausea.
- Rare: Paradoxical bronchospasm, tachycardia, pruritis, dizziness.
Pregnancy & Lactation
- Pregnancy: Category C. Use only if benefits outweigh risks.
- Lactation: Caution advised as it may be excreted in breast milk.
Precautions & Warnings
Potential for serious breathing problems similar to those associated with salmeterol. Not approved for asthma treatment.
Overdose Effects
Symptoms include excessive β-adrenergic stimulation effects like tachycardia, hypertension, arrhythmias, nervousness, and more. Cardiac arrest and death may occur.
Storage Conditions
Keep capsules in their blister pack and store in a cool, dry place. Keep out of reach of children.
