Indications
Olanap ODT is used for the acute and long-term treatment of schizophrenia and related psychoses, targeting both positive symptoms (such as delusions, hallucinations, and disordered thinking) and negative symptoms (such as social withdrawal and flattened affect). It is also prescribed for acute manic or mixed episodes in bipolar disorder, with or without psychotic features and rapid cycling.
Pharmacology
Olanap ODT, a powerful antipsychotic medication, works by blocking a range of receptors including serotonin (5HT) and dopamine (D1-D5), as well as histamine and muscarinic receptors. While the exact mechanism is unknown, it’s believed that the drug’s efficacy in schizophrenia stems from its ability to antagonize serotonin type 2 (5HT2) and dopamine receptors. After oral administration, olanzapine reaches peak plasma levels within 5-8 hours, and food does not affect its absorption.
Dosage and Administration
For the treatment of schizophrenia, the initial dose is 5-10 mg per day. Depending on the clinical response, the dose may be adjusted, with a maximum of 20 mg daily. For bipolar mania, an initial dose of 15 mg daily is recommended, with adjustments based on the patient’s condition, and the maximum dose is also 20 mg daily. Olanzapine can be taken without regard to meals.
Contraindications
Olanap ODT should not be used by patients who are hypersensitive to any of its ingredients, those with a history of narrow-angle glaucoma, or those with certain cardiac conditions. It is also contraindicated in pregnant women and lactating mothers.
Side Effects
Common side effects of Olanap ODT include somnolence, weight gain, increased appetite, elevated glucose levels, and dizziness. Less common but serious side effects may include Parkinsonism, orthostatic hypotension, and dry mouth. It may also cause photosensitivity reactions.
Pregnancy & Lactation
Olanap ODT should only be used in pregnancy if the potential benefits outweigh the risks. It should not be used during breastfeeding, as it is not known whether olanzapine is excreted in human breast milk.
Precautions
Use caution when administering Olanap ODT to patients with seizure history, bone marrow suppression, or impaired liver function. Elderly patients and those on medications that increase QT intervals should be closely monitored. Driving and operating heavy machinery should be avoided if sedative effects are present.
Use in Special Populations
- Children: Safety and efficacy in individuals under 18 years have not been established.
- Elderly (65+ years): Start with a dose of 5 mg/day.
- Renal and Hepatic Impairment: The starting dose is also 5 mg/day.
