Indications
Nutridex is used for treating fluid and carbohydrate depletion. Primarily, it helps replenish water deficiencies and is recommended when there is no significant electrolyte loss. Dehydration occurs when fluid losses are not adequately replaced, which can happen in conditions like coma, dysphagia, or with elderly individuals who may not drink enough water. Nutridex serves as a quick source of metabolizable nutrients, with 1 liter of a 5% solution providing about 170 calories. Glucose solutions are also used in combination with calcium bicarbonate and insulin to manage hyperkalemia in emergencies. During the treatment of diabetic ketoacidosis, glucose solutions can be used after addressing hyperglycemia, alongside continued insulin infusion. Nutridex is also an ideal medium for the slow intravenous delivery of various medications.
Pharmacology
Dextrose, a form of glucose, is provided as a 5% solution in water to replenish fluids and supply carbohydrates via intravenous infusion. It is commonly used to treat conditions like low blood sugar (hypoglycemia), insulin shock, or dehydration. Dextrose 5% in water can also offer nutritional support for patients who are unable to eat due to illness, injury, or other medical conditions.
Dosage Guidelines
10% Dextrose Solution:
The dosage is determined by the patient’s clinical condition, age, and body surface area. Typically, 20-30 ml/kg is administered rapidly to restore normal circulation. Further fluid therapy is guided by laboratory results.
Oral Powder:
- Adults: 10-20 gm as a single dose, with a repeat dose after 10 minutes if necessary.
- Children (over 2 years): 10-20 gm as a single dose, with the possibility of a second dose after 10 minutes if needed.
Administration Instructions
- Inspect the infusion set and solution before use.
- Tear off the Eurocap’s protective cover gently.
- Grip the Eurocap (not the bottle) and attach the administration set.
- Open the flow regulator fully and hold the set at the top (away from the membrane vent).
- Insert the spike into the Eurocap and firmly attach the set to the needle.
- Allow fluid to flow to the needle tip before closing.
- Clean the venipuncture site with antiseptic, insert the needle, and secure the puncture site.
- Adjust the drip speed and start the infusion.
Contraindications
Do not use Nutridex if you are allergic to dextrose. Before starting treatment, inform your doctor if you have diabetes, breathing issues, electrolyte imbalances, kidney or liver problems, food or drug allergies, or if you receive regular blood transfusions.
Side Effects
Possible side effects include:
- Severe burning, pain, or swelling at the IV site
- Redness, warmth, oozing, or bleeding at the injection site
- Fever or persistent cough
- High blood sugar, headache, weakness, or memory issues
- Low potassium, anxiety, sweating, pale skin, or irregular heart rate
Pregnancy & Lactation
Nutridex is classified as FDA pregnancy category C. It is unknown if it could harm an unborn baby, so consult your doctor if you are pregnant or planning to become pregnant. It is also unclear if it passes into breast milk. Speak to your doctor before using this medication if you are breastfeeding.
Precautions & Warnings
Monitor blood glucose levels closely, especially for diabetic patients who may require concurrent insulin. Do not mix Nutridex with whole blood as it can cause hemolysis or clumping. Discontinue infusion if rigors develop. Avoid using Nutridex if the solution is cloudy, contains particles, or has passed its expiration date.
