Indications:
Nitrosol SR is indicated for the prophylaxis of angina pectoris. It is not suitable for aborting acute anginal episodes due to its onset of action.
Pharmacology:
Nitroglycerin induces relaxation of vascular smooth muscle, resulting in vasodilation of peripheral arteries and veins. This effect reduces left ventricular end-diastolic pressure (preload) and systemic vascular resistance (afterload).
Dosage & Administration:
The dosage of Nitrosol SR should be adjusted based on individual patient response and anginal pain severity. For adults, take one 2.6 mg sustained-release tablet or capsule in the morning and evening on an empty stomach.
Interactions:
Nitrosol SR may potentiate the antihypertensive effects of vasodilators, calcium antagonists, and beta-adrenergic blockers. Concomitant use with tricyclic antidepressants and alcohol may lead to increased blood pressure. Combining with phosphodiesterase type 5 (PDE5) inhibitors like sildenafil, vardenafil, and tadalafil can cause hypotension.
Contraindications:
Contraindicated in patients hypersensitive to nitroglycerin, other organic nitrates/nitrites, or excipients. Also contraindicated in acute myocardial infarction, marked anaemia, head trauma, cerebral haemorrhage, or closed angle glaucoma.
Side Effects:
Common side effects include initial headaches which typically subside with continued use. Adjusting dosage may alleviate persistent headaches. Other side effects may include tachycardia, postural hypotension, syncope, cyanosis, and methaemoglobinaemia.
Pregnancy & Lactation:
Safety during pregnancy and lactation has not been established. Use only if considered essential by a physician.
Precautions & Warnings:
Use with caution in patients predisposed to closed-angle glaucoma. Avoid abrupt discontinuation; taper dosage gradually over several days if long-term therapy is stopped.
Overdose Effects:
Overdose symptoms may include peripheral vasodilation, decreased blood pressure, and reflex tachycardia. Monitor cardiac function and consider fluid replacement or vasopressors in severe cases.
