Description

Indications

Nepanil 0.1% Sterile Ophthalmic Suspension: This medication is indicated for managing post-operative ocular pain and inflammation, particularly following cataract surgery. It helps inhibit surgery-induced miosis and prevents post-operative cystoid macular edema (CME).

Nepanil 0.3% Sterile Ophthalmic Suspension: This formulation is designed to alleviate post-operative pain and inflammation associated with cataract procedures.

Pharmacology

Nepafenac is classified as a nonsteroidal anti-inflammatory prodrug (NSAID). Upon instillation into the eye, it effectively penetrates the cornea and is metabolized by ocular tissue hydrolase into Amfenac, a potent NSAID. Amfenac is believed to inhibit the cyclooxygenase enzyme, essential for the synthesis of prostaglandins.

Dosage & Administration

Nepafenac 0.1% Sterile Ophthalmic Suspension:

  • For post-operative pain and inflammation: Administer 1 drop three times daily, starting one day before cataract surgery and continuing through the first two weeks post-operatively.
  • For surgery-induced miosis: Instill 1 drop three times daily, beginning one day before the surgery and on the day of the procedure.
  • To prevent post-operative CME: Instill 1 drop three times daily, starting one day prior to surgery and continuing on the day of surgery through the first six weeks post-operative.

Nepafenac 0.3% Sterile Ophthalmic Suspension:

  • For post-operative pain and inflammation: Instill 1 drop once daily, beginning one day before cataract surgery and continuing for the first two weeks post-operatively. An additional drop should be administered 30 to 120 minutes before surgery.

Contraindications

Nepanil is contraindicated in patients with a known hypersensitivity to any ingredients in the formulation or to other NSAIDs.

Side Effects

Common side effects may include a sensation of a foreign body in the eye, crusting along the lid margins, ocular discomfort, and ocular hyperemia.

Pregnancy & Lactation

Use in Pregnancy: Adequate and well-controlled studies in pregnant women are lacking. Nepafenac should only be used during pregnancy if the potential benefits justify any potential risks to the fetus.

Use in Lactation: The excretion of this drug in human milk is not established. Caution is advised when administering Nepafenac to nursing mothers.

Precautions & Warnings

Use Nepanil with caution in patients who have bleeding tendencies or who are taking medications that may prolong bleeding time.

Use in Special Populations

Use in Children: The safety and effectiveness of Nepafenac in pediatric patients under 18 years of age have not been established.

Use in Elderly Patients: No significant differences in safety or efficacy have been observed between elderly and younger patients.
