Indications
This medication is prescribed for treating topical bacterial infections and demonstrates efficacy in various conditions such as atopic, contact, and stasis dermatitis, as well as neurodermatitis, eczema, and anogenital pruritus. Additionally, it can serve as a supportive therapy in certain pyodermas like impetigo, especially when used alongside systemic antibiotic treatment.
Pharmacology
Bacitracin works by hindering bacterial cell wall synthesis and exhibits activity against a wide range of gram-positive bacteria (e.g., staphylococci, streptococci) and some gram-negative species (e.g., Neisseria, Haemophilus influenzae). Often included in topical formulations, it acts as a broad-spectrum antibacterial agent.
Neomycin, an aminoglycoside akin to gentamicin, binds to bacterial ribosome 30S subunits, thereby impeding protein synthesis and disrupting DNA synthesis. It is effective against many gram-negative aerobes and certain strains of staphylococci.
Dosage & Administration
For ointment application, cleanse the area gently before use, removing any debris such as pus or crusts. Apply two to three times daily on the affected skin area. The antibiotic powder, suitable for superficial skin infections, should be lightly dusted on the affected area up to four times daily.
Interaction
Bacitracin Zinc: Heightened risk of nephrotoxicity when co-administered with other nephrotoxic drugs. Potential enhancement of neuromuscular-blocking agents’ action.
Neomycin Sulphate: Combined nephrotoxic and neurotoxic effects with certain drugs like aminoglycosides, bacitracin, and vancomycin. Enhanced toxicity when used with potent diuretics. Possible interference with the absorption of other medications.
Contraindications
Avoid if there’s hypersensitivity to any components of the medication.
Side Effects
Nebanol is generally well-tolerated; however, allergic reactions are rare.
Pregnancy & Lactation
The safety of this preparation during pregnancy and lactation hasn’t been established definitively. Thus, it should be used during pregnancy only if the potential benefits outweigh the potential risks to the fetus.
Precautions & Warnings
Avoid using Bacitracin & Neomycin antibiotic powder on large open wounds or severely injured skin. Use caution with Neomycin-containing preparations in cases of otitis external with a perforated eardrum to mitigate the risk of ototoxicity.
Patients considering self-medication with a topical anti-infective for deep wounds or serious burns should first consult a physician. If minor skin injuries persist or worsen, discontinue use of the topical anti-infective preparation and consult a physician; it should not be used for longer than a week unless directed by a physician.
Use in Special Populations
The ointment is suitable for children at adult doses but should be administered at a reduced dose for infants. It’s not recommended for neonates.
