Indications
Mucomist is recommended as an adjunctive therapy for conditions involving thick, viscous, or purulent mucus. It is effective in managing:
- Chronic Respiratory Diseases: Including chronic obstructive pulmonary disease (COPD), emphysema with bronchitis, chronic asthmatic bronchitis, and bronchiectasis.
- Acute Respiratory Diseases: Such as asthma with mucus plugging, bronchitis, bronchopneumonia, tracheobronchitis, bronchiolitis, pulmonary complications of cystic fibrosis, and post-surgical pulmonary issues.
Pharmacology
Acetylcysteine, the active ingredient in Mucomist, acts as a mucolytic agent by breaking down disulfide bonds in mucoproteins, thereby reducing mucus viscosity. It also offers an antisecretory effect, aiding in the clearance of respiratory passages and easing breathing. Additionally, Acetylcysteine has antioxidant properties, neutralizing harmful free radicals and supporting glutathione production, which is crucial for combating oxidative stress in respiratory conditions.
Dosage & Administration
- Effervescent or Dispersible Tablets:
- Adults and Children Over 6 Years: Take one 600 mg effervescent tablet daily (preferably in the evening). Acute treatment should last 5 to 10 days; for chronic conditions, treatment may extend for several months as directed by a physician. Dissolve the tablet in a small amount of water, stir if needed, and consume directly from the glass.
- Effervescent Granules:
- Adults and Children Over 6 Years: Use one 200 mg sachet or two 100 mg sachets 2-3 times daily.
- Children (2-6 Years): Administer one 100 mg sachet 2-4 times daily, based on age.
The duration of treatment should be 5 to 10 days for acute cases, with extended use for chronic conditions as recommended by a healthcare provider. Dissolve the sachets in water and drink the solution directly.
- Nebuliser Solution:
- Adults: Inhale 5-10 ml of a 10% or 20% solution via nebuliser every 6-8 hours as needed.
- Children:
- 1-11 Months: Inhale 1-2 ml of 20% solution or 2-4 ml of 10% solution every 6-8 hours as needed.
- 12 Months to 11 Years: Inhale 3-5 ml of 20% solution or 6-10 ml of 10% solution every 6-8 hours as needed.
- Below 12 Years: Use 5-10 ml of 10% or 20% solution every 6-8 hours as needed.
For Diagnostic Bronchograms: Nebulise 1-2 ml of 20% or 2-4 ml of 10% solution 2-3 times prior to the procedure.
Nebulisation Tent or Croupette: Requires large volumes, up to 300 ml, to maintain a heavy mist. Administer as needed for intermittent or prolonged treatments.
Direct Instillation: Administer 1-2 ml of a 20% solution hourly or every 1-4 hours for tracheostomy care.
Interactions
Mucomist may reduce the effectiveness of certain antibiotics, such as ampicillin, tetracycline, macrolides, cephalosporins, aminoglycosides, and amphotericin. To avoid interaction, take Mucomist and antibiotics 2 hours apart. It may also enhance the effects of nitroglycerine on platelet aggregation and vasodilation.
Contraindications
Do not use Mucomist if you have a known hypersensitivity to Acetylcysteine or phenylketonuria, due to its aspartame content.
Side Effects
Mucomist can cause side effects, although not everyone will experience them. Rare severe reactions include anaphylactic shock and severe skin reactions. Common side effects may include headache, increased heart rate, stomatitis, itching, urticaria, nausea, vomiting, abdominal pain, fever, lowered blood pressure, diarrhea, and tinnitus.
Pregnancy & Lactation
Consult a physician or pharmacist before using Mucomist during pregnancy or lactation.
Precautions & Warnings
Monitor patients with bronchial asthma closely; discontinue use if bronchospasm occurs. Exercise caution in those with a history of peptic ulcer. Note that each dose contains 156.9 mg of sodium, which should be considered for patients on a sodium-restricted diet. A sulphureous odor is normal and does not indicate product alteration. Avoid mixing Mucomist with other medications.
Overdose Effects
Overdosing on Mucomist may result in nausea, vomiting, or diarrhea.
