Pharmacology

Migrex contains Tolfenamic Acid, a potent cyclooxygenase (COX) inhibitor belonging to the fenamate group. By inhibiting the synthesis of key inflammatory mediators like prostaglandins (PGs) and thromboxane B2, it helps reduce inflammation, pain, and swelling associated with conditions like migraines. Tolfenamic acid also has direct antagonistic effects on prostaglandin receptors.

• Absorption: Quickly absorbed through the gastrointestinal tract.
• Peak Plasma Concentration: 60-90 minutes.
• Bioavailability: 85%.
• Distribution: 99% protein binding.
• Half-Life: 2 hours.
• Metabolism: Metabolized in the liver, undergoing enterohepatic circulation.
• Excretion: Mainly excreted via urine (90%) and feces.

Dosage & Administration

• Adults:
  • Acute migraine attacks: 200 mg at the onset of symptoms, with a second dose after 1-2 hours if needed.
  • Mild to moderate pain: 100-200 mg 3 times daily.
  • Renal impairment: Dose adjustments may be necessary.
  • Severe renal impairment: Avoid use.
• Children: Not recommended for pediatric use, as dosage guidelines have not been established.

Take Migrex with food and drink plenty of water during or immediately after meals.

Interactions

Migrex may interact with the following medications:

• Increased absorption: Metoclopramide, Magnesium hydroxide.
• Decreased absorption: Aluminium hydroxide.
• Risk of bleeding: When combined with anticoagulants or other NSAIDs.
• Increased plasma concentration: Lithium, Methotrexate, cardiac glycosides.
• Nephrotoxicity risk: With ACE inhibitors, Ciclosporin, Tacrolimus, or diuretics.

Contraindications

• Active gastrointestinal bleeding or ulcers.
• History of gastrointestinal perforation or bleeding due to previous NSAID use.
• Severe heart failure.

Side Effects

Possible side effects may include:

Pregnancy & Lactation

Migrex should not be used during pregnancy, especially in the third trimester, unless considered essential by your doctor. Tolfenamic acid can pass into breast milk in very low concentrations. It’s best to avoid NSAIDs during breastfeeding unless necessary.

Precautions & Warnings

Overdose Effects

Symptoms of overdose may include:

• Dizziness, drowsiness, and convulsions.
• Acute renal failure and liver damage may occur in significant poisoning.

Treatment is symptomatic and should be managed under medical supervision.
