Indications
Maxrin Hydrochloride effectively treats the functional symptoms of Benign Prostatic Hyperplasia (BPH), offering relief and improved urinary function.
Pharmacology
Tamsulosin, an alpha1 adrenoceptor blocking agent, selectively targets alpha1 A adrenoceptors in the prostate, leading to muscle relaxation in the bladder neck and prostate. This relaxation enhances urine flow rate and reduces BPH symptoms. Absorption of Tamsulosin hydrochloride capsule 0.4mg is nearly complete (90%) under fasting conditions, reaching peak concentration within four to five hours. It is extensively metabolized in the liver and excreted mainly through urine.
Dosage & Administration
For optimal results, take Tamsulosin Hydrochloride 0.4 mg (one capsule) daily after a meal at night. The dosage may increase to Tamsulosin Hydrochloride 0.8 mg (two capsules) once daily after 2 to 4 weeks, if needed. Swallow the capsule whole with water (about 150 ml) while standing or sitting. Avoid crushing or chewing the capsule to maintain its modified release.
Interaction
While administering other alpha1-adrenoceptor antagonists concurrently may lead to hypotensive effects, Maxrin shows no interactions with atenolol, enalapril, or nifedipine. Cimetidine may increase Maxrin plasma levels, while frusemide may decrease them. However, these changes usually remain within the normal range. In vitro studies suggest no hepatic metabolism interactions with various drugs, except for diclofenac and warfarin, which may increase Maxrin elimination rate.
Contraindications
Tamsulosin hydrochloride should not be used in patients with hypersensitivity to it or a history of orthostatic hypotension. It is also contraindicated in severe hepatic insufficiency.
Side Effects
Adverse reactions reported with Maxrin include dizziness, abnormal ejaculation, and occasionally headache, asthenia, postural hypotension, and palpitations.
Pregnancy & Lactation
Avoid using Tamsulosin during pregnancy and lactation.
Precautions & Warnings
Patients may experience transient postural symptoms after the first dose. It’s advised to avoid driving or operating machinery if dizziness occurs. Before initiating therapy, patients should be evaluated for conditions mimicking BPH symptoms. Regular examinations, including digital rectal examination and Prostate Specific Antigen (PSA) testing, are recommended during treatment.
Overdose Effects
Acute hypotension may occur in case of overdose, necessitating cardiovascular support. Measures such as lying down, volume expanders, and vasopressors may be required. Renal function should be monitored, and supportive care provided.
