Indications
MarinCal-D is indicated for the treatment and prevention of:
- Osteoporosis
- Osteomalacia
- Tetany
- Hypoparathyroidism
- Osteogenesis disorders
It also serves as a calcium and vitamin D supplement in cases of inadequate dietary intake during childhood, rickets, pregnancy, lactation, and for elderly patients. Additional indications include pancreatitis and as a phosphate binder in chronic renal failure.
Composition
Each tablet contains:
- Calcium Carbonate USP: 1250 mg (coral source), equivalent to 500 mg elemental calcium & Vitamin D USP: 200 IU.
- DX Tablet: Calcium Carbonate USP 1500 mg (coral source), equivalent to 600 mg elemental calcium & Vitamin D USP: 400 IU.
- Effervescent Tablet: Calcium Carbonate (coral source) USP 1500 mg, equivalent to 600 mg elemental calcium and Vitamin D3 400 IU as Cholecalciferol USP.
Pharmacology
Maintaining sufficient calcium levels in the blood is crucial for numerous bodily functions, as deficiencies can lead to significant health issues. Clinical studies indicate that calcium is vital in preventing and treating osteoporosis and associated fractures. Vitamin D is essential for bone health as it enhances calcium absorption in the gastrointestinal tract and stimulates bone formation. Research shows that calcium and vitamin D work synergistically to support bone growth and prevent osteoporosis and fractures. The calcium in this preparation is sourced from coral, ensuring better absorption due to its similarity to human bone composition.
Dosage & Administration
Take one tablet once or twice daily with plenty of water, or as directed by your physician. For optimal absorption, it’s recommended to take the tablet on a full stomach. Effervescent tablets should be dissolved in a glass of water, stirred, and consumed immediately.
Interactions
MarinCal-D may interact with thiazide diuretics, systemic corticosteroids, ion exchange resins, and certain antibiotics, affecting calcium absorption. Regular monitoring of serum calcium levels is advised during concurrent use, particularly with digoxin therapy. Avoid taking calcium products within two hours of consuming foods high in oxalic or phytic acids to prevent absorption issues.
Contraindications
Do not use MarinCal-D if you have:
- Hypercalcemia or hyperparathyroidism
- Hypercalciuria or nephrolithiasis
- Hypersensitivity to any ingredients
- Severe renal insufficiency
Side Effects
Calcium carbonate may irritate the gastrointestinal tract and cause constipation. Although hypercalcemia from calcium alone is rare, it can occur in patients with chronic renal failure. Other potential side effects include allergic reactions, irregular heartbeats, nausea, vomiting, decreased appetite, dry mouth, drowsiness, and skin rash following vitamin D administration.
Pregnancy & Lactation
During pregnancy, daily intake should not exceed 1500 mg of calcium and 600 IU of cholecalciferol (vitamin D). Avoid overdoses, as high levels can adversely affect fetal development. MarinCal-D can be used safely during breastfeeding.
Precautions & Warnings
Monitor serum calcium levels and renal function during long-term treatment, especially in elderly patients or those on cardiac glycosides and diuretics. In case of hypercalcemia or renal impairment, reduce the dose or discontinue treatment. Caution is advised for patients with sarcoidosis and those who are immobilized due to an increased risk of hypercalcemia.
Overdose Effects
Overdose can lead to hypervitaminosis D and hypercalcemia, with symptoms including nausea, vomiting, abdominal pain, fatigue, and severe cases may result in cardiac arrhythmias or coma. Treatment involves discontinuing calcium intake and monitoring renal function.
