Indications
Patadin Eye Drops are formulated to alleviate the signs and symptoms of allergic conjunctivitis, including itchy, watery, red, and swollen eyes and eyelids. It is effective for conditions such as vernal keratoconjunctivitis, vernal keratitis, blepharitis, blepharoconjunctivitis, and giant papillary conjunctivitis.
Lopadine Max Nasal Spray acts as an H1 receptor antagonist, providing relief from the symptoms of seasonal allergic rhinitis in adults and children aged six years and older.
Lopadine Max Tablets are indicated for the management of allergic rhinitis, characterized by swelling of the nasal membranes due to allergies from pollen and mold spores, as well as allergic skin conditions. These tablets help alleviate allergy symptoms such as skin rashes, swelling, hives (itchy, painful bumps), itching, sneezing, and runny or congested noses.

Pharmacology
Olopatadine is an antihistamine that blocks histamine’s action in the body, thereby alleviating allergy symptoms. It effectively treats sneezing, itching, runny nose, and other nasal manifestations of allergies.
Mode of Action: Olopatadine functions as an anti-allergic agent through multiple mechanisms. It stabilizes mast cells, inhibiting the immunologically-triggered release of histamine. Additionally, olopatadine selectively antagonizes the histamine H1 receptors, blocking the signaling pathway responsible for releasing inflammatory mediators like tryptase, prostaglandin D2, TNF-alpha, and pro-inflammatory cytokines.
Pharmacokinetics:
- Absorption: Following twice-daily intranasal administration of Olopatadine Nasal Spray, peak plasma concentrations are typically reached within 30 minutes to 1 hour.
- Distribution: Olopatadine exhibits moderate protein binding (approximately 55%) in human serum, independent of drug concentration across a range of 0.1 to 1000 ng/mL.
- Metabolism: Olopatadine undergoes minimal metabolism, with at least six minor metabolites detectable in plasma.
- Elimination: The plasma elimination half-life of olopatadine ranges from 8 to 12 hours, primarily excreted via urine.
Dosage
For Eye Drops:
- 0.1%: One drop in each affected eye, twice daily at intervals of 6 to 8 hours.
- 0.2%: One drop in the affected eye, once daily.
- 0.7%: One drop in each affected eye, once daily.
For Nasal Spray:
- Administer Olopatadine Nasal Spray intranasally only.
- Adults and Adolescents (12 years and older): Two sprays per nostril twice daily.
- Children (6 to 11 years): One spray per nostril twice daily.
For Tablets:
- Adults: Take one tablet (5 mg) twice daily, in the morning and before bed. Dosage may be adjusted based on disease severity, age, or symptoms.
- Children (7 years and older): Take one tablet (5 mg) twice daily, in the morning and before bed. If a dose is missed, take it as soon as remembered, but skip it if it’s near the next dose. Do not take two doses simultaneously. Do not discontinue without consulting your doctor.

Administration
How to Use the Nasal Spray:
- Gently shake the bottle and remove the dust cover.
- Hold the spray with your index and middle fingers on either side of the nozzle and your thumb underneath the bottle. Press down until a fine mist appears. If it’s your first use or hasn’t been used for a week, pump the nasal applicator several times until a fine mist is visible.
- Blow your nose gently to clear the nostrils.
- Close one nostril, insert the nasal applicator into the open nostril, tilt your head slightly forward, and keep the spray upright. Inhale through your nose while firmly pressing down the applicator to release a spray.
- Exhale through your mouth.
- Repeat in the same or opposite nostril for subsequent doses.
Cleaning the Nasal Spray: Clean the nasal spray at least once a week by following these steps:
- Remove the dust cover.
- Gently detach the nasal applicator.
- Wash the applicator and dust cover in warm water.
- Shake off excess water and let them dry in a normal location, avoiding additional heat.
- Reattach the applicator and dust cover.

Interaction
Formally conducted drug-drug interaction studies for Lopadine Max Nasal Spray are lacking. Since olopatadine is primarily eliminated through renal excretion, interactions with liver enzyme inhibitors are unlikely. Drug interactions involving P450 inhibition and plasma protein binding are also not expected.
Contraindications
Lopadine Max is contraindicated for individuals with known hypersensitivity to any component of this product. The olopatadine 5 mg tablet is not recommended if you are allergic to it. Always inform your doctor of your current medications and pre-existing medical conditions to ensure safety.
Side Effects
While rare, some users may experience headaches. Other potential side effects include asthenia, blurred vision, burning or stinging sensations, cold symptoms, dry eyes, foreign body sensation, hyperemia, hypersensitivity, keratitis, eyelid swelling, nausea, pharyngitis, itching, rhinitis, sinusitis, and altered taste.
Pregnancy & Lactation
Studies in rats and rabbits indicate that olopatadine is not teratogenic. However, there are no well-controlled studies in pregnant women. This medication should only be used during pregnancy if the potential benefits outweigh the risks to the fetus. It is unclear whether topical ocular administration could result in significant systemic absorption detectable in human breast milk, so caution is advised when administering olopatadine to nursing mothers.
Precautions & Warnings
Patients should refrain from wearing contact lenses if their eyes are red. Lopadine Max ophthalmic solution should not be used for contact lens-related irritation. The preservative benzalkonium chloride in Patadin may be absorbed by soft contact lenses. If you wear soft contact lenses and your eyes are not red, wait at least 10 minutes after instilling Patadin eye drops before inserting them. Discontinue treatment if an allergic reaction occurs.
Before initial use, prime the Lopadine Max Nasal Spray by releasing five sprays or until a fine mist is visible. If not used for more than seven days, re-prime by releasing two sprays. Avoid spraying Lopadine Max Nasal Spray in the eyes.
Olonex Tablets should be used as directed by your doctor. Take them at the same time daily to maintain a consistent level of the medication in your body. Avoid exceeding the recommended dose and do not discontinue without consulting your doctor, as symptoms may recur.
Use in Special Populations
Geriatric Use: No overall differences in safety or effectiveness have been observed in elderly patients.
Overdose Effects
