Composition
Each milliliter of this suspension contains 100 IU (equivalent to 3.50 mg) of Insulin Aspart (rDNA) BP, comprising 30% soluble Insulin Aspart and 70% protamine-crystallized Insulin Aspart.
Description
Glyset Mix combines protamine and soluble insulin, resulting in a sterile suspension that contains 30% soluble Glyset Mix and 70% protamine-crystallized Glyset Mix. This formulation acts as an effective blood glucose-lowering agent, offering a quicker onset of action and an intermediate duration. Glyset Mix is structurally similar to regular human insulin, differing only by a single amino acid substitution (proline replaced by aspartic acid at position B28), and is developed through advanced recombinant DNA technology.
Pharmacology
The primary function of Insulin Aspart is to regulate glucose metabolism. It binds to insulin receptors on muscle and fat cells, lowering blood glucose levels by promoting cellular uptake of glucose while simultaneously inhibiting hepatic glucose output.
Dosage
Dosage of Insulin Aspart is personalized and should be determined by a physician based on individual patient needs. For patients with type 2 diabetes, it can be administered as monotherapy or in conjunction with oral antidiabetic medications when blood glucose levels are insufficiently controlled. The recommended initial dose for type 2 diabetes is 6 IU at breakfast and 6 IU at dinner (evening meal). Alternatively, it can be started with a once-daily dose of 12 IU at dinner. If a patient requires more than 30 units, transitioning to twice-daily dosing with equal amounts at breakfast and dinner is suggested. For those experiencing recurrent daytime hypoglycemia, consider splitting the morning dose into morning and lunchtime doses (three times daily).
If hypoglycemia occurs, do not increase the dosage within three days. Weekly adjustments can be made until target HbA1c levels are achieved. Patients with type 1 diabetes typically require 0.5 to 1.0 IU/kg/day, and Insulin Aspart can fulfill this requirement. When switching from biphasic human insulin to Insulin Aspart, begin with the same dosage and adjust according to individual requirements. Insulin Aspart is suitable for elderly patients, though limited experience exists with its combination with oral antidiabetic drugs in those over 75 years.

Interactions
Several medications can influence glucose metabolism and may necessitate dosage adjustments. The following substances may decrease the requirements for Glyset Mix: oral antidiabetic drugs, ACE inhibitors, disopyramide, fibrates, fluoxetine, monoamine oxidase inhibitors, propoxyphene, pentoxifylline, salicylates, and sulfonamide antibiotics. Conversely, the following substances may increase Glyset Mix requirements: thiazides, glucocorticoids, thyroid hormones, beta-sympathomimetics, growth hormone, and danazol. Additionally, beta-blockers, clonidine, lithium salts, and alcohol may either enhance or diminish the blood glucose-lowering effects of insulin.
Contraindications
Insulin Aspart should not be used:
- During episodes of hypoglycemia
- In patients with a known hypersensitivity to Insulin Aspart or any of its components
Side Effects
Potential side effects of Glyset Mix include hypoglycemia, allergic reactions, injection site reactions, lipodystrophy, pruritus, and rash.
Pregnancy & Lactation
Classified as Pregnancy Category B, there are no known restrictions on the use of Insulin Aspart during breastfeeding. Insulin treatment in nursing mothers is unlikely to affect the infant; however, dosage adjustments may be necessary.

Overdose Effects
While a specific overdose definition for insulin does not exist, excessive dosages relative to the patient’s needs may lead to hypoglycemia. Mild episodes can be treated with oral glucose or sugary products. For severe hypoglycemia resulting in unconsciousness, administer glucagon (0.5 to 1 mg) intramuscularly or subcutaneously. If the patient does not respond within 10 to 15 minutes, intravenous glucose should be provided. Once consciousness is regained, oral carbohydrates should be given to prevent relapse.
