Indications for Gabarol (Pregabalin) Tablets

Gabarol (Pregabalin) is used to manage and treat various neuropathic conditions, including:

• Neuropathic pain associated with diabetic peripheral neuropathy
• Postherpetic neuralgia (nerve pain following shingles)
• Adjunctive therapy for partial-onset seizures in patients aged 1 month and older
• Fibromyalgia (widespread muscle pain)
• Neuropathic pain resulting from spinal cord injury

Gabarol CR (Controlled Release) is specifically indicated for:

• Neuropathic pain due to diabetic peripheral neuropathy
• Postherpetic neuralgia

Pharmacology of Gabarol

Pregabalin, the active ingredient in Gabarol, is a synthetic derivative of gamma-aminobutyric acid (GABA). Although it does not directly bind to GABAA, GABAB, or benzodiazepine receptors, Pregabalin works by attaching to the alpha2-delta subunit of voltage-gated calcium channels in the central nervous system. This mechanism is thought to contribute to its effectiveness in treating neuropathic pain and managing seizures.

Dosage Guidelines

• For Diabetic Peripheral Neuropathy (DPN): Start with 50 mg three times a day (150 mg/day), with a gradual increase up to 300 mg/day based on effectiveness and tolerance. For Gabarol CR, begin with 165 mg once daily, increasing to 330 mg within one week.
• For Postherpetic Neuralgia: Start with 75 mg twice a day or 50 mg three times a day, with a gradual increase to 300 mg/day. In case of insufficient pain relief, increase the dose to 600 mg/day.
• For Fibromyalgia: Begin with 75 mg twice a day (150 mg/day), gradually increasing to 450 mg/day.
• For Neuropathic Pain with Spinal Cord Injury: Start with 75 mg twice a day (150 mg/day), and increase up to 600 mg/day based on individual response.

Dosing adjustments are based on individual needs and should be done under medical supervision.

Administration of Gabarol

• Oral Administration: Gabarol can be taken with or without food. The controlled-release (CR) formulation should be taken after an evening meal. Do not chew, crush, or split the tablets. If a dose is missed, follow the instructions carefully depending on the timing of the next dose.
• Gradual Discontinuation: When stopping Gabarol, reduce the dosage gradually over a period of at least one week to prevent withdrawal symptoms.

Drug Interactions

• Gabarol is unlikely to cause significant pharmacokinetic interactions with other medications.
• Always inform your healthcare provider about all other medications you are taking to avoid potential interactions.

Contraindications and Warnings

• Gabarol is contraindicated in individuals who are hypersensitive to Pregabalin or any of its components.
• Angioedema and severe allergic reactions, including difficulty breathing, may occur. If this happens, discontinue the medication immediately.
• Suicidal thoughts and behavior have been linked with antiepileptic drugs like Gabarol. Close monitoring is recommended, especially during the initial stages of treatment.

• Dry mouth, edema, and blurred vision
• Weight gain, and changes in attention or concentration

In children, weight gain and increased appetite are commonly observed.

Pregnancy & Lactation

• Pregabalin should only be used during pregnancy if the potential benefits outweigh the risks. Always consult your doctor before use.
• Breastfeeding is not recommended while using Gabarol, as small amounts of Pregabalin are passed through breast milk.

Precautions

• Respiratory depression may occur if Gabarol is combined with other CNS depressants.
• Peripheral edema may develop, particularly in patients taking antidiabetic medications (e.g., thiazolidinediones).
• Gradually tapering the dosage over 1 week is essential when discontinuing Gabarol to avoid withdrawal effects.

Overdose Symptoms and Treatment
