Indications
This medication is designed for the ongoing management of asthma, particularly suitable for patients who may benefit from a combination of a long-acting β2-agonist and an inhaled corticosteroid. It’s appropriate for those inadequately managed with inhaled corticosteroids and ‘as needed’ short-acting β2-agonists, or for patients already stabilized on both inhaled corticosteroids and long-acting β2-agonists.
Pharmacology
Salmeterol Xinafoate acts as a selective long-acting beta-2 agonist, effectively addressing asthma and various airway obstructions. Fluticasone Propionate, a glucocorticoid corticosteroid, targets lung inflammation with minimal systemic effects at recommended doses. Together, they provide comprehensive symptom control and enhanced lung function, making treatment easier for patients requiring both therapies.
Dosage
For inhalers: Adults and adolescents (12+ years) may take 2 puffs of 25 µg Salmeterol and 50/125/250 µg Fluticasone Propionate twice daily. Children (4-12 years) should take 2 puffs of 25 µg Salmeterol and 50 µg Fluticasone Propionate twice daily. Inhalation capsules for adults and adolescents start at Salmeterol 50 µg & Fluticasone 100/250 µg twice daily, with a max of 50 µg & 500 µg twice daily.
Administration
Proper inhaler technique is crucial for effective delivery of the medication. Follow these steps: shake the inhaler, exhale fully, place the mouthpiece in your mouth, inhale deeply while pressing down on the canister, hold your breath, and rinse your mouth after use.
Interactions
Avoid both selective and non-selective β-blockers unless necessary in asthma patients. Monitor interactions with strong CYP3A4 inhibitors like ketoconazole, which may elevate Fluticasone Propionate levels.
Contraindications
This medication should not be used by individuals with hypersensitivity to salmeterol, fluticasone, or their components, nor in acute asthma episodes needing immediate intervention.
Side Effects
Common side effects may include tremors, palpitations, and headaches from Salmeterol, and hoarseness or candidiasis from Fluticasone. Most side effects are mild and can decrease with continued use.
Pregnancy & Lactation
The inhalation capsule should only be used during pregnancy or breastfeeding if the potential benefits outweigh risks, given the limited data on its effects.
Precautions & Warnings
Initiate treatment cautiously, avoiding use during exacerbations. Patients should always have rescue medication on hand. Monitor for potential asthma-related adverse events and paradoxical bronchospasm.

Overdose Effects
While no specific overdose data exists for this inhaler, signs of overdose for Salmeterol include tachycardia and hypertension, while Fluticasone may lead to temporary adrenal suppression.
