Indications
Flamex, available in both oral and topical forms, is used to treat a wide range of painful conditions. The oral dosage is effective for managing Rheumatoid Arthritis, Osteoarthritis, Gouty Arthritis, Juvenile Polyarthritis, Ankylosing Spondylitis, Synovitis, Low Back Pain, Dysmenorrhoea, Fever, Migraine, Soft Tissue Injuries, as well as Dental and Musculoskeletal Pain.
The Flamex Topical Gel (5%) contains Ibuprofen, a member of the non-steroidal anti-inflammatory drugs (NSAIDs) class, which helps reduce pain, inflammation, and fever. This gel is specifically formulated for joint and muscle pain, including conditions such as back pain, rheumatic pain, sprains, strains, sports injuries, and non-severe arthritic conditions. It’s also beneficial for treating nerve pain (neuralgia).

Pharmacology
Ibuprofen, the active ingredient in Flamex, is renowned for its potent anti-inflammatory, analgesic, and antipyretic properties. It works by inhibiting cyclooxygenase enzymes, which play a key role in the production of prostaglandins, the compounds responsible for pain and inflammation. This dual action—both peripheral and central—helps alleviate pain, reduce swelling, and improve joint movement in conditions like arthritis.
Dosage & Administration
- Oral Dosage:
- Adults: The initial dose is 400 mg, taken three times daily. The total daily dose should not exceed 2400 mg.
- Children:
- 3-6 months (over 5 kg): ½ tsp (2.5 ml) three times a day (maximum 30 mg/kg daily).
- 6 months-1 year: ½ tsp (2.5 ml) 3-4 times daily (maximum 30 mg/kg daily).
- 1-4 years: 1 tsp (5 ml) three times daily (maximum 30 mg/kg daily).
- 4-7 years: 1½ tsp (7.5 ml) three times daily (maximum 30 mg/kg daily).
- 7-10 years: 2 tsp (10 ml) three times daily (maximum 2.4 gm daily).
- 10-12 years: 3 tsp (15 ml) three times daily (maximum 2.4 gm daily).
- For juvenile rheumatoid arthritis, a dosage of 30-40 mg/kg of body weight per day, divided into 3-4 doses, may be prescribed.
- Topical Gel:
- Adults: Apply 4 to 10 cm of gel (equivalent to 50-125 mg of Ibuprofen) to the affected area. Gently massage until fully absorbed. Apply no more than four times a day, with a minimum interval of four hours between doses.
- Use in Children: Ibuprofen 5% gel is not recommended for children under 12 years of age.

Contraindications
Flamex should not be used by individuals who are allergic to Ibuprofen or other NSAIDs, or those with active peptic ulcers or severe gastrointestinal conditions.
Side Effects
Common side effects may include upset stomach, vomiting, heartburn, and nausea. If these persist or worsen, consult a healthcare provider.
Pregnancy & Lactation
The use of Ibuprofen during pregnancy, especially in the third trimester, should be avoided unless the benefits outweigh the risks. It is not recommended for nursing mothers due to potential adverse effects on the baby. Always consult with your doctor before use during pregnancy or breastfeeding.
Precautions & Warnings
- Flamex should be used with caution in individuals with a history of gastrointestinal issues (e.g., ulcers or bleeding).
- Regular ocular monitoring is advised for patients on long-term Flamex therapy, as changes in eye function have been observed.
- Patients with systemic lupus erythematosus or asthma may be more susceptible to side effects and should use Flamex under medical supervision.
- Caution is advised when prescribing to individuals with a history of allergic reactions to NSAIDs.
