Indications
Duvent is prescribed for managing:
- Seasonal and perennial allergic rhinitis
- Urticaria (hives)
Pharmacology
Duvent features Rupatadine, a long-acting, non-sedative H1-receptor antagonist. It also inhibits platelet-activating factor (PAF), which contributes to inflammation and bronchoconstriction. This dual action provides enhanced therapeutic effects compared to standard antihistamines. Rupatadine further aids in reducing mast cell degranulation and suppressing the release of cytokines like tumor necrosis factor-alpha (TNF-α).
Dosage & Administration
- Adults and Adolescents (12 years and older): 10 mg once daily, with or without food.
- Children (2 to 11 years):
- Weighing 25 kg or more: 5 ml oral solution once daily.
- Weighing 10 kg to less than 25 kg: 2.5 ml oral solution once daily.
Interactions
- With Medications: Use cautiously with ketoconazole, erythromycin, statins, CNS depressants, and alcohol.
- With Food: Avoid grapefruit and grapefruit juice.
Contraindications
- Known hypersensitivity to Rupatadine or any components of the formulation.
Side Effects
- Common: Asthenia, dizziness, drowsiness.
- Less Common: Increased appetite, arthralgia, back pain, impaired concentration, constipation, cough, diarrhea, dry throat, epistaxis, fever, gastrointestinal discomfort, irritability, malaise, myalgia, nasal dryness, nausea, oropharyngeal pain, rash, thirst, vomiting, weight gain.
- Rare: Palpitations, tachycardia.
Pregnancy & Lactation
- Pregnancy: Use only if benefits outweigh potential risks, as no clinical data is available.
- Lactation: Not recommended due to lack of information on excretion in breast milk.
Precautions & Warnings
- Caution in patients with QT interval prolongation, uncorrected hypokalemia, or proarrhythmic conditions.
- Use cautiously in the elderly.
- Not recommended for patients with renal or hepatic impairment.
Use in Special Populations
- Elderly: No specific dose adjustment required, but use with caution.
- Children: Safety and efficacy not established in those under 12 years.
- Renal or Hepatic Insufficiency: Not recommended due to lack of clinical data.
Overdose Effects
- Most common reaction: Somnolence. Manage with symptomatic and supportive care.
