Indications
Vitamin D3 deficiency can arise due to limited sunlight exposure or a diet lacking in Vitamin D3. This essential nutrient plays a crucial role in facilitating calcium and phosphate absorption, vital for maintaining healthy bones and teeth, and preventing conditions like rickets, osteomalacia, and osteoporosis. Additionally, adequate Vitamin D3 intake is essential during pregnancy and breastfeeding to reduce the risk of pre-eclampsia and support infant development. Vitamin D3 also supports immune system function.
Pharmacology
The active form of Vitamin D3, Calcitriol, functions by binding to Vitamin D receptors (VDRs) found in various body tissues. Vitamin D3, being fat-soluble, has a half-life of approximately 50 days. After absorption in the small intestine, it binds to specific globulins and undergoes metabolism in the liver to form 25-hydroxy Vitamin D3 (Calcidiol). Further hydroxylation in the kidneys produces 1,25-dihydroxy Vitamin D3 (Calcitriol), which enhances calcium absorption. Unused Vitamin D3 is stored in fat and muscle tissues and eliminated through feces and urine.
Dosage & Administration
- Capsule (Adults):
- Treatment of Vitamin D3 deficiency: 40,000 IU once weekly for 7 weeks. Maintenance therapy ranges from 1,400-2,000 IU/day.
- Prevention of Vitamin D3 deficiency: 20,000 IU every 4 weeks, with adjustments as needed.
- Addition to osteoporosis therapy: 20,000 IU monthly.
- Capsule (Children 12-18 years):
- Treatment of Vitamin D3 deficiency: 20,000 IU once every 2 weeks for 6 weeks.
- Prevention of Vitamin D3 deficiency: 20,000 IU every 6 weeks.
- Film-coated tablet: 1,000 IU (1-2 tablets) daily, with food or within 1 hour after a meal.
- Oroflash or chewable tablets: 1,000-2,000 IU daily, taken with food.
- Syrup: Dosage varies by age and deficiency risk.
Interaction
Several medications can lead to Vitamin D3 deficiency by interfering with its absorption and metabolism. These include antacids containing magnesium, digoxin, thiazide diuretics, and corticosteroids like prednisolone.
Contraindications
