Indications

Cilosta serves to alleviate symptoms associated with intermittent claudication, characterized by an enhanced walking distance.

Pharmacology

Derived from the quinolinone group, Cilostazol, the active component of Cilosta, acts by selectively inhibiting cellular phosphodiesterase III (PDE III). This action impedes cAMP degradation, fostering increased cAMP levels in platelets and blood vessels. Consequently, platelet aggregation is hindered, and vasodilation occurs.

Dosage & Administration

For optimal efficacy, Cilostazol is typically prescribed at a dosage of 100 mg twice daily, to be taken at least thirty minutes before or two hours after meals. In cases where coadministration with certain medications such as Fluconazole, Ketoconazole, or others is necessary, a reduced dosage of 50 mg twice daily may be considered.

Pediatric use: The safety and effectiveness of Cilostazol in pediatric patients have yet to be established. Please consult a healthcare professional before use.

Interactions

Studies on pharmacokinetics have revealed notable increases in systemic exposure to Cilosta and its major metabolites when coadministered with Omeprazole or Erythromycin. Additionally, population pharmacokinetic studies have indicated elevated concentrations of Cilosta in patients concurrently receiving Diltiazem.

Contraindications

Cilostazol is contraindicated in individuals with congestive heart failure of any degree of severity and in those with known or suspected hypersensitivity to any of its constituents.

Side Effects

Common side effects include headache, diarrhea, vomiting, leg cramps, and rash, while less frequent ones encompass anorexia and edema.

Pregnancy & Lactation

The safety of Cilostazol during pregnancy has not been adequately studied. Transfer of Cilostazol into milk has been observed in animal studies. Therefore, nursing mothers should either discontinue breastfeeding or discontinue Cilostazol, considering the potential risk to infants.

Precautions & Warnings

Exercise caution when administering Cilosta to patients with any degree of heart failure. Currently, there is insufficient data regarding the concurrent use of Cilosta and Clopidogrel.
