Brodil Respirator Solution

Indications

Brodil is used as a bronchodilator to treat various respiratory conditions, including:

• Asthma
• Chronic Bronchitis
• Emphysema
• Other airway obstruction-related disorders

Pharmacological Information

Salbutamol is a synthetic sympathomimetic drug primarily acting on beta-2 adrenergic receptors. It promotes bronchodilation by stimulating these receptors in the smooth muscles of the bronchial tubes, leading to muscle relaxation and improved lung function. This effect can be measured using spirometric tests.

Dosage & Administration

Salbutamol Tablet or Syrup:

• Children:
  • 2-6 years: 2.5 ml syrup, 3-4 times daily
  • 6-12 years: 5 ml syrup, 3-4 times daily
  • Over 12 years: 5-10 ml syrup, 3-4 times daily or 2-4 mg tablet, 3-4 times daily
• Adults: 2-4 mg tablet, 3-4 times daily (Maximum single dose: 8 mg)

Salbutamol Respiratory Solution:

• Adults & Elderly:
  • 0.5 ml – 1.0 ml, up to 4 times a day. Maximum daily dose: 40 mg (under medical supervision). Dilute 0.5-1 ml with sterile saline (2-4 ml total volume) for nebulization.
  • For undiluted administration, 2.0 ml of the solution can be nebulized for 3-5 minutes until bronchodilation is achieved.
• Children (under 12 years):
  • 0.5 ml, diluted to 2.0-4.0 ml saline. Some may require up to 1.0 ml. Treatment may be repeated up to four times a day.

Method-2 (Continuous Administration):

• 1-2 ml solution diluted to 100 ml with saline for continuous nebulization. Oxygen may be added if there is a risk of hypoxia.

Salbutamol Nebulizer Solution:

• Adults & Elderly: 2.5 mg to 5 mg, up to 4 times a day. Daily max: 40 mg (under strict medical supervision).
• Children (under 12 years): 2.5 mg, up to 4 times a day. Higher doses (up to 5 mg) may be used if necessary.

Salbutamol Injection:

• Adults: Dilute 5 ml (1000 mcg/ml) of Salbutamol IV infusion solution into 500 ml saline or dextrose solution. Administer at 3-20 micrograms per minute. Start at 5 mcg/min and adjust as needed.
• Children & Adolescents: No established dosage for those under 18 years.

Salbutamol Inhalation Capsule:

• Adults: For relief of bronchospasm, inhale one or two 200 mcg capsules every 4-6 hours. For maintenance therapy, use one 200 mcg capsule every 4-6 hours. Maximum two capsules if needed.
• Children (4+ years): One 200 mcg capsule for acute bronchospasm. Use 1-2 capsules every 4-6 hours for maintenance or before exercise.

Exercise-Induced Asthma:

• Adults: 400 mcg, 15-30 minutes before exercise.
• Children: 200 mcg, 15-30 minutes before exercise.

Side Effects

Brodil may cause side effects such as fine muscle tremors (especially in hands), palpitations, muscle cramps, tachycardia, headaches, and peripheral vasodilation, particularly with high doses.

Pregnancy & Lactation

Brodil should only be used during pregnancy if the potential benefits outweigh any risks to the fetus. It is unclear if the drug is excreted in breast milk. Due to potential tumorigenicity in animal studies, the decision to continue breastfeeding or stop the medication should be made in consultation with a healthcare professional.

Precautions & Warnings

Use Brodil cautiously in individuals with:

• Hyperthyroidism
• Cardiovascular disease
• Hypertension or vascular disorders
• Diabetes
• Glaucoma or those on antihypertensive therapy

High doses may lead to hypokalemia, increasing the risk of arrhythmias. Tachyphylaxis (diminishing response) may occur with prolonged high-dose use.

Overdose Effects

Overdose symptoms may include angina, headaches, nausea, vomiting, and tremors. A cardioselective beta-blocker is the preferred treatment for overdose, though caution is necessary for patients with a history of bronchospasm.

Therapeutic Class
