Pharmacology
Citalopram, the active ingredient in Arpolax, is a bicyclic phthalane derivative that functions as a selective serotonin reuptake inhibitor (SSRI). It minimally affects the reuptake of noradrenaline, dopamine, and GABA. This selective inhibitory action contributes to its antidepressant effects, while it shows low affinity for various receptors, including 5-HT1A, 5-HT2A, D1, D2, adrenergic, histamine H1, and muscarinic receptors.
Dosage & Administration
For Depressive Illness:
- Adults: Start with 20 mg daily, taken as a single dose either in the morning or evening. The dosage may be increased to a maximum of 60 mg daily if needed.
- Elderly: Maximum dosage is 40 mg daily.
- Children: Not recommended.
For Panic Disorder:
- Adults: Begin with 10 mg daily, increasing to 20 mg after 7 days. The usual dosage ranges from 20 to 30 mg daily, with a maximum of 60 mg.
- Elderly: Maximum dosage is 40 mg daily.
- Children: Not recommended.

Interactions
Citalopram should not be used alongside MAOIs, serotonin agonists, neuroleptics, cimetidine, alcohol, ketoconazole, itraconazole, or macrolide antibiotics.
Contraindications
Citalopram is contraindicated for patients experiencing a manic phase.

- Taste disturbances
- Increased salivation

Pregnancy & Lactation
- Pregnancy: There are no adequate and well-controlled studies in pregnant women. Citalopram should only be used during pregnancy if the potential benefits justify the risks to the fetus.
- Lactation: Citalopram is excreted in human breast milk. Decisions regarding breastfeeding during Citalopram therapy should consider the risks for the infant and the benefits for the mother.
Precautions & Warnings
Arpolax should be used cautiously in individuals with:
- Epilepsy
- Ongoing electroconvulsive therapy
- History of mania
- Cardiac disease
- Diabetes mellitus
- Angle-closure glaucoma
- History of bleeding disorders
- Hepatic and renal impairment
- Pregnancy and breastfeeding
Overdose Effects
While doses as high as 2000 mg have not shown significant cardiovascular effects, overdose may result in respiratory depression and convulsions.
