Indications
Arbitel AM is prescribed for managing hypertension either as a standalone treatment or alongside other antihypertensive medications. It is also suitable for patients who may require multiple drugs to meet their blood pressure targets.

Pharmacology
Arbitel AM combines Telmisartan (an angiotensin II receptor blocker) and Amiodipine (a calcium-channel blocker) to provide effective blood pressure management.
- Telmisartan blocks the vasoconstrictor effects of angiotensin II, reducing blood pressure and promoting vasodilation without affecting the pulse rate.
- Amiodipine reduces peripheral vascular resistance, leading to improved blood flow and reduced blood pressure.
This combination works synergistically to lower blood pressure effectively and is highly effective in treating hypertensive patients.
Dosage & Administration
Initial Therapy: Start with 40/5 mg once daily if controlling blood pressure with a single medication is insufficient. For more significant blood pressure reduction, 80/5 mg may be used daily.
Add-on Therapy: For patients inadequately controlled on either amiodipine or telmisartan alone, Arbitel AM can be introduced for enhanced results.
Replacement Therapy: Patients on separate doses of amiodipine and telmisartan may switch to Arbitel AM for convenience.
Contraindications
- Known hypersensitivity to Telmisartan or Amiodipine.
- Pregnancy and lactation.
- Severe hepatic impairment, hypotension, or cardiogenic shock.
Side Effects
Some common side effects include dizziness, headache, nausea, edema, palpitations, and abdominal discomfort. Serious side effects like hypotension and chest pain may also occur.
Pregnancy & Lactation
- First trimester: Category C
- Second and third trimesters: Category D.
This medication should be discontinued during pregnancy, especially if breastfeeding due to potential risks to the infant.
Precautions & Warnings
- Hypotension: Correct any volume depletion before starting treatment.
- Renal or Hepatic Impairment: Use with caution, especially in patients with kidney or liver issues.
- Avoid using alongside ACE inhibitors and angiotensin receptor blockers.
Use in Special Populations
- Pediatric: Safety not established for those under 18.
- Geriatric: Not recommended for patients aged 75 and above without proper medical assessment.
Overdose Effects
- Telmisartan: Overdose may cause hypotension and dizziness. Symptomatic treatment recommended.
- Amiodipine: Can lead to excessive vasodilation and hypotension. Supportive care, including fluids or vasopressors, may be required.
