Indications
Ornova is prescribed for the treatment of:
- Gastric and duodenal ulcers
- Reflux esophagitis
- Prevention of ulcer recurrence during low-dose aspirin or NSAID therapy
- Adjunct treatment for Helicobacter pylori eradication in cases of gastric ulcers, duodenal ulcers, MALT lymphoma, idiopathic thrombocytopenic purpura, post-endoscopic resection of early-stage gastric cancer, or Helicobacter pylori gastritis.

Pharmacology
Vonoprazan is a potassium-competitive acid blocker (P-CAB) that does not require activation by acid. It inhibits gastric acid secretion by competitively blocking H+, K+-ATPase in gastric parietal cells. With its strong basicity, it binds to acid-producing sites for a prolonged duration, providing effective protection against mucosal damage in the gastrointestinal tract.
Dosage
- Gastric and Duodenal Ulcer: 20 mg once daily for 6-8 weeks.
- Reflux Esophagitis: 20 mg once daily for up to 8 weeks.
- Maintenance Therapy for Reflux Esophagitis: 10 mg daily, increasing to 20 mg if needed.
- Prevention During Low-Dose Aspirin or NSAID Use: 10 mg daily.
- Helicobacter Pylori Eradication: 20 mg Vonoprazan, 750 mg amoxicillin, and 200 mg clarithromycin twice daily for 7 days.

Administration
Ornova can be taken with or without food.
Contraindications
- Hypersensitivity to Vonoprazan or its ingredients.
- Co-administration with atazanavir, nelfinavir, or rilpivirine.
Side Effects
Possible side effects include diarrhea, constipation, drug hypersensitivity, jaundice, liver function changes, rash, nausea, and abdominal discomfort.
Pregnancy & Lactation
Use during pregnancy only when the benefits outweigh the risks. Avoid using Vonoprazan in nursing mothers unless absolutely necessary.
Precautions & Warnings
- Monitor the patient regularly during treatment, especially for long-term use.
- Use lower doses or discontinue treatment if the risk of recurrence is low.
- Caution is advised for elderly patients and those with liver or kidney impairment.
Special Populations
- Elderly: Use cautiously due to reduced hepatic or renal function.
- Children: Not recommended for use in children under 18.
- Renal Impairment: No dose adjustment for mild to moderate impairment; avoid in severe impairment.
- Hepatic Impairment: Avoid in moderate to severe liver dysfunction.
Overdose Effects
There is no specific antidote for an overdose. Symptomatic and supportive treatment is recommended.
