Description
Hypnofast is a sleep-inducing medication known for its rapid onset and short duration of action. It also provides anxiolytic, anticonvulsant, and muscle-relaxant effects.
Pharmacology
Benzodiazepines like midazolam function by enhancing the action of gamma-aminobutyric acid (GABA), a key inhibitory neurotransmitter in the central nervous system. By increasing GABA activity, benzodiazepines induce sedation, relax skeletal muscles, and promote sleep, anesthesia, and amnesia. These drugs bind to the benzodiazepine site on GABA-A receptors, enhancing the effects of GABA by increasing the frequency of chloride channel openings. While these receptors are found in various tissues, they are predominantly present in the central nervous system.
Dosage & Administration
Oral Dosage:
- Adults: 7.5-15 mg daily.
- Elderly and Debilitated Patients: Recommended dose is 7.5 mg.
- Premedication: Administer 15 mg of Midazolam 30-60 minutes prior to the procedure.
Intravenous Administration:
- Endoscopic or Cardiovascular Procedures: Initial dose of approximately 2.5 mg for healthy adults; reduce to 1-1.5 mg for elderly or severely ill patients.
- Induction of Anesthesia: Dose of 10-15 mg.
Intramuscular Administration:
- Adults: 0.07-0.1 mg/kg body weight; typical dose is about 5 mg.
- Children: 0.15-0.20 mg/kg.
- Elderly and Debilitated Patients: 0.025-0.05 mg/kg.
Rectal Administration for Children:
- Preoperative Sedation: Administer 0.35-0.45 mg/kg rectally 20-30 minutes before general anesthesia induction.

Interaction
Hypnofast may enhance the sedative effects of neuroleptics, tranquilizers, antidepressants, sleep aids, analgesics, anesthetics, antipsychotics, anxiolytics, antiepileptic drugs, and sedative antihistamines.
Contraindications
Midazolam is contraindicated in patients with severe respiratory insufficiency, severe hepatic insufficiency, myasthenia gravis, sleep apnea syndrome, or known hypersensitivity to benzodiazepines or any component of the product.
Side Effects
Initial side effects may include daytime drowsiness, confusion, fatigue, headache, and muscle weakness, which typically resolve with continued use. Following parenteral administration, fluctuations in vital signs, including respiratory depression, apnea, and variations in blood pressure and pulse rate, may occur.
Pregnancy & Lactation
Midazolam should be avoided during pregnancy unless no safer alternative is available. It is also excreted in breast milk and should not be given to breastfeeding mothers.
Precautions & Warnings
Administer Hypnofast IV very slowly to avoid complications.
Overdose Effects
Severe overdose can lead to coma, areflexia, cardiorespiratory depression, and apnea. Overdose effects can be managed with the benzodiazepine antagonist flumazenil.
