Indications
Brezofil SR is indicated for the treatment of:
- Bronchial asthma
- Bronchospasm
- Chronic obstructive pulmonary disease (COPD)
- Pulmonary diseases with a spastic bronchial component

Pharmacology
Brezofil SR contains Doxophylline, a unique bronchodilator that features a dioxolane group, differentiating it from Theophylline. It selectively inhibits phosphodiesterase 4, leading to relaxation of bronchial smooth muscle. Unlike Theophylline, Doxophylline has reduced affinity for adenosine A1 and A2 receptors, contributing to its enhanced safety profile. It also inhibits platelet-activating factor (PAF) and reduces leukotriene production.
Dosage & Administration
- Elderly: 200 mg tablet, two or three times daily.
- Adults: 400 mg tablet, two or three times daily or as directed by a physician.
- Children:
- >12 years: 10 ml syrup or 200 mg tablet, two or three times daily.
- 6-12 years: 6-9 mg/kg body weight, twice daily (e.g., 3 ml for a 10 kg child).
If a daily dose of 400 mg is required, use the Doxophylline SR tablet once daily as prescribed.

Interactions
Avoid administering Brezofil SR with other xanthine preparations. Limit caffeine intake from food and beverages. Use caution when combining with sympathomimetics like ephedrine. Various medications such as erythromycin and cimetidine may alter the clearance of xanthine derivatives, impacting plasma levels.
Contraindications
Brezofil is contraindicated for individuals with a known hypersensitivity to Doxophylline or other xanthine derivatives, as well as those with acute myocardial infarction, hypotension, or during lactation.
Side Effects
Common side effects may include nausea, vomiting, and epigastric pain. Rare effects can include insomnia, fatigue, and seizures. Overdose may lead to severe cardiac arrhythmias and tonic-clonic seizures.
Pregnancy & Lactation
While animal studies show no adverse effects on prenatal and postnatal growth, the use of Doxophylline during pregnancy should be assessed on a case-by-case basis due to insufficient human data. It is contraindicated in nursing mothers.
Precautions & Warnings
Caution is advised for patients with cardiac issues, hypertension, or severe hypoxemia. Monitor dosage and plasma levels in those with liver disease or on medications affecting xanthine clearance.
Overdose Effects
Severe overdose symptoms may include arrhythmias and seizures. There is no specific antidote; symptomatic treatment should be initiated for cardiovascular collapse.
