Composition

Each milliliter of suspension contains 100 IU (equivalent to 3.50 mg) of Insulin Aspart (rDNA) BP, comprising 30% soluble Insulin Aspart and 70% protamine-crystallized Insulin Aspart.

Description

Glyset Mix is a sterile suspension of a human insulin analog that consists of 30% soluble Glyset Mix and 70% protamine-crystallized Glyset Mix. As a blood glucose-lowering agent, it offers a quicker onset of action with an intermediate duration. Glyset Mix is similar to regular human insulin, differing only by a single amino acid substitution, where proline is replaced by aspartic acid at position B28. This product is produced through recombinant DNA technology.

Pharmacology

The main function of Insulin Aspart is to regulate glucose metabolism. It binds to insulin receptors on muscle and fat cells, effectively reducing blood glucose levels by facilitating cellular glucose uptake while simultaneously inhibiting glucose release from the liver.

Dosage

The dosage of Insulin Aspart is personalized and should be determined by a physician based on individual patient needs. In individuals with type 2 diabetes, Insulin Aspart may be used alone or in combination with oral antidiabetic medications when blood glucose levels remain uncontrolled with oral therapy. The typical starting dose for type 2 diabetes patients is 6 IU at breakfast and 6 IU at dinner. Alternatively, it can be initiated with a single daily dose of 12 IU at dinner. If using a once-daily regimen, it is often advised to transition to a twice-daily schedule when the total reaches 30 units, dividing the dose equally between breakfast and dinner. Should twice-daily dosing lead to recurrent daytime hypoglycemia, the morning dose may be split into morning and lunchtime doses, resulting in a three-times-daily regimen.

If hypoglycemia occurs within three days, the dosage should not be increased. Adjustments can be made weekly until the target HbA1c is achieved. For individuals with type 1 diabetes, the insulin requirement typically ranges from 0.5 to 1.0 IU/kg/day, which Insulin Aspart can fully or partially fulfill. When transitioning a patient from biphasic human insulin to Insulin Aspart, initiate with the same dose and regimen, then adjust according to individual needs. Insulin Aspart is suitable for elderly patients; however, there is limited data on its use with oral antidiabetic drugs in those over 75 years old.

Interaction

Several medications can influence glucose metabolism and may necessitate dosage adjustments. The following substances may reduce the need for Glyset Mix: oral antidiabetic medications, angiotensin-converting enzyme (ACE) inhibitors, disopyramide, fibrates, fluoxetine, monoamine oxidase inhibitors, propoxyphene, pentoxifylline, salicylates, and sulfonamide antibiotics. Conversely, the following substances may increase the requirement for Glyset Mix: thiazides, glucocorticoids, thyroid hormones, beta-sympathomimetics, growth hormone, and danazol. Additionally, beta-blockers, clonidine, lithium salts, and alcohol may either enhance or diminish the blood glucose-lowering effects of insulin.

Contraindications

Insulin Aspart is contraindicated:

  • During episodes of hypoglycemia.
  • In patients with hypersensitivity to Insulin Aspart or any of its excipients.

Side Effects

Common side effects associated with Glyset Mix include hypoglycemia, allergic reactions, injection site reactions, lipodystrophy, pruritus, and rash.

Pregnancy & Lactation

Glyset Mix is classified as Pregnancy Category B. There are no restrictions on the use of Insulin Aspart during breastfeeding. However, dosage adjustments may be necessary.

Precautions & Warnings

Monitoring and Dose Adjustment: Regular blood glucose monitoring is essential for all patients receiving insulin therapy. Any adjustments to the Glyset Mix regimen should be made cautiously and under medical supervision.

Use in Special Populations:

  • Renal Impairment: A reduced dose may be necessary.
  • Hepatic Impairment: A reduced dose may be necessary.

Overdose Effects

While a specific overdose definition for insulin does not exist, excessive doses can lead to hypoglycemia in stages. Mild hypoglycemic episodes can be treated with oral glucose or sugary products. For severe hypoglycemia resulting in unconsciousness, glucagon (0.5 to 1 mg) should be administered intramuscularly or subcutaneously. If the patient does not respond to glucagon within 10 to 15 minutes, intravenous glucose should be administered. Once consciousness is regained, oral carbohydrates are recommended to prevent relapse.
