Indications
Cinarex is a topical antibiotic prescribed for treating external bacterial infections of the eye caused by susceptible bacteria. It is effective against:
- Gram-Positive Bacteria: Includes Staphylococcus aureus, Staphylococcus epidermidis, Streptococcus pneumoniae, and certain non-hemolytic Streptococcus species.
- Gram-Negative Bacteria: Includes E. coli, Pseudomonas aeruginosa, Enterobacter aerogenes, Klebsiella spp., Proteus mirabilis, Proteus vulgaris, Haemophilus influenzae, Morganella morganii, Acinetobacter calcoaceticus, Providentia, Serratia, Salmonella spp., and some Neisseria strains.
Pharmacology
Cinarex contains Tobramycin, an aminoglycoside antibiotic. It works by entering sensitive bacterial cells through active transport, binding to the 30S and partially to the 50S ribosomal subunits. This binding inhibits protein synthesis and disrupts the genetic code transcription, leading to bacterial cell death. The exact mechanism is not fully understood, and additional effects on membrane permeability may also contribute.
Dosage & Administration
- Ophthalmic Ointment:
- Mild to Moderate Infections: Apply a small amount 2-3 times daily into the conjunctival sac(s).
- Severe Infections: Apply a small amount 3-4 times daily until improvement is noted, then gradually reduce the dose.
- Ophthalmic Solution:
- Mild to Moderate Infections: Instill 1 or 2 drops into the affected eye(s) every 4 hours.
- Severe Infections: Instill 2 drops into the affected eye(s) every hour until improvement is seen.
- Pediatric Use: Safety and effectiveness in children under 1 year have not been established.
Use as directed by your physician.
Interactions
Use Cinarex cautiously if the patient is also receiving neuromuscular blocking agents or ototoxic drugs.
Contraindications
Cinarex should not be used by individuals who are hypersensitive to Tobramycin or any component of the formulation.
Side Effects
Common side effects include localized ocular toxicity, conjunctival redness, and hypersensitivity reactions such as itching and swelling of the eyelids. If these effects persist or worsen, consult your doctor.
Pregnancy & Lactation
Cinarex is classified as Pregnancy Category B. There are no comprehensive studies in pregnant women; use only if the benefits justify the potential risk to the fetus. It may be excreted in human milk, so consider discontinuing nursing or the drug based on a risk-benefit assessment.
Precautions & Warnings
Topical aminoglycosides may cause minor sensitivity reactions in some patients. Discontinue use if sensitivity occurs. Prolonged use may lead to overgrowth of non-susceptible organisms and fungi.
Overdose Effects
Overdose symptoms are similar to the side effects described above. Seek medical advice if an overdose is suspected.

Storage Conditions
Store in a cool, dry place (below 25°C) and protect from light. Keep out of reach of children. Use within 4 weeks of opening.
